摘　　要：目的测定2型糖尿病(T2DM)、糖耐量受损(IGT)患者血清胰岛素和C肽水平,分析糖代谢和胰岛B细胞的功能。方法 T2DM 93例、IGT 41例分组,以36名健康体检者为对照,标准馒头餐进行胰岛素、C肽释放试验,测定各个时段胰岛素、C肽水平。结果 T2DM组和IGT组患者空腹胰岛素和C肽水平正常或略高,餐后分泌曲线低平,分泌高峰延迟至2 h,不同病程糖尿病患者胰岛素和C肽水平差异较大。IGT患者存在高胰岛素血症和明显的胰岛素抵抗状态。结论化学发光法进行胰岛素、C肽释放实验,可用于糖尿病患者管理,判断患者胰岛B细胞功能,指导临床治疗。Objective To test the serum insulin and C-peptide levels of patients with type 2 diabetes mellitus（T2DM） and impaired glucose tolerance（IGT）, and analyze carbohydrate metabolism and the function of insular B cells.Methods A total of 93 patients with T2 DM, 41 patients with IGT and 36 healthy controls were investigated. The insulin and C-peptide levels were tested in various periods after standard steamed bread meal. Results In the T2 DM group and IGT group, the fasting insulin and C-peptide levels were normal or slightly higher, and the postprandial secretion curve was low and flat, and the secretion peak delayed to 2 hours.The insulin and C-peptide levels were quite different in differ-ent courses of diabetes. In addition, the IGT patients existed of high insulin levels and obvious insulin resistance status.Conclusion The insulin and C-peptide release test by chemiluminescenceimmunoassay can be used for the management of diabetics andjudgmentof pancreatic islet B cell function, and guiding clinical treatment.
