摘　　要：目的对比分析左心室发育不良型法洛四联症(tetralogy of fallot,TOF)一期手术和两期手术的临床效果。方法 95例左心室发育不良型TOF患者分为一期手术组(50例)和两期手术组(45例),一期手术组采用新型左心室扩大技术对左心室发育不良型TOF行根治术;两期手术组第一次行体-肺动脉分流术,第二次行根治术。结果 2组死亡率无差异,一期手术组手术手术时间短、住院时间短、住院费用低、切口愈合不良发生率低、术后身高体重增长快,低心排综合症(low cardiac output syndrome,LCOS)肾功能不全发生率高。结论一期手术治疗左心室发育不良型TOF总体效果优于两期手术,但要注意LCOS的防治及肾功能的保护。Objective To compare clinical effects in Fallot （TOF） of one-stage operation and two- stage operation. Methods 95 cases of TOF with small left ventricular were divided into one-stage opera- tion group （50 cases） and two-stage operation group （45 cases）. A new left ventricular expansion tech- nique was used in one-stage operation group. In two-stage operation group, the body-pulmonary artery shunt operation was followed by a radical operation. Results There was no difference in mortality between the two groups. Compared with two-stage operation group, the one-stage operation group had decreased operation time, hospital stay time, hospital cost, and incidence of wound healing, rapidly increased height and weight. But the low cardiac output syndrome and renal failure rate increased in one-stage operation group. Conclusion One-stage operation has better overall effect than two-stage operation. But close attention should be paid to the prevention and treatment of LCOS and the protection of renal function.
