外固定术联合封闭负压引流对胫骨开放性骨折的应用效果

摘　　要：目的：探讨外固定术联合封闭负压引流治疗胫骨开放性骨折的效果。方法选取2007年2月至2015年2月胫骨开放性骨折患者54例，分为观察组28例和对照组26例，两组患者均行外固定术治疗，对照组术后实施传统局部换药，观察组术后给予封闭负压引流术，观察两组创面愈合、骨折愈合、膝关节及踝关节活动度。结果观察组创面愈合优良率（96.4％）及骨折愈合优良率（100％）均高于对照组，差异有统计学意义（P ﹤0.05）。随访6个月，观察组骨痂 X 线评分、BMD 值、膝关节和踝关节活动度均优于对照组，差异有统计学意义（P ﹤0.05）。结论封闭负压引流能显著提高外固定术治疗效果，降低创面感染发生率，促进骨折愈合和关节功能恢复。Objective To investigate the clinical effect of external fixation combined with closed negative pressure drainage on open tibia fracture. Methods From February 2007 to February 2015,54 patients with open tibia fracture were selected,and were divided into observation group（28 cases）and control group（26 cases）,the patients of the two groups were treated with external fixation,the control group received the traditional local dressing change after surgery,while the observation group was treated by closed negative pressure drainage. The rate of wound healing and fracture healing,the activity of knee joint and ankle joint between the two groups were observed. Results The surface healing fine rate of the observer group was 96. 4% ,and the fracture healing fine rate was 100% ,which were higher than those of the control group,the differences were significant（P ﹤ 0. 05）. Six months follow-up,callus X-ray scores,BMD value,knee and ankle ac-tivity of the observation group were better than that of the control group,the differences were significant （P ﹤ 0. 05）. Conclusions Closed negative pressure drainage can significantly improve the effect of ex-ternal fixation for the treatment,reduce the incidence of wound infection,improve fracture healing and the recovery of joint function.
