摘 要:目的探讨核心蛋白多糖对大鼠骨骼肌钝挫伤后致骨髓炎中瘢痕增生的抑制作用。方法60只SPF级成年雄性Wistar大鼠,采用自行改良设计的钝挫伤装置造模,造成大鼠股外侧肌群钝挫伤。显露股骨及股骨大转子,向髓腔内注射金黄色葡萄球菌菌液,建立骨髓炎模型。按随机数字表法分为对照组(A组)和核心蛋白多糖干预组(B组),每组30只。A组于术后0,5,10,15,20,25d局部肌肉注射50“磷酸盐缓冲液(PBS),B组给予10μg/ml核心蛋白多糖局部肌肉注射。两组分别于术后7,14,28d取骨骼肌组织检测,采用HE染色评价慢性骨髓炎中骨骼肌瘢痕化的程度,Westernblot检测肿瘤坏死因子-β1(TGF—β1)、磷酸化Smad3和I型胶原细胞因子的表达。结果HE染色观察发现,与A组比较,B组术后14d及28d组织纤维化程度明显减轻。术后7d,A组TGF-81、磷酸化Smad3和I型胶原蛋白表达水平分别为0.152±0.031,0.439±0.035,0.714±0.116;B组分别为0.145±0.039,0.446±0.036,0.712±0.078。B组与A组比较,差异无统计学意义(P〉0.05)。术后14d,A组TGF-β1、磷酸化Smad3和I型胶原蛋白表达水平分别为0.263±0.015,1.082±0.017,1.171±0.138;B组分别为0.136±0.004,0.155±0.040,0.217±0.025;术后28d,A组TGF—β1、磷酸化Smad3和I型胶原蛋白表达水平分别为0.754±0.042,1.197±0.086,1.825±0.044;B组分另4为0.307±0.006,0.193±0.028,0.256±0.057。B组较A组的表达水平明显降低(P〈0.05)。结论局部肌肉注射核心蛋白多糖可以有效抑制大鼠骨髓炎中骨骼肌瘢痕化程度。Objective To investigate the mechanism of decorin inhibiting scat" formation in a rat osteomyelitis model by investigating the expressions of tumor growth factor (TGF)-β1, phosphpo-Smad3 (p-Smad3) and collagen I in skeletal muscle. Methods Sixty SPF rats were subjected to a muscular contusion injury with a self-improved blunt device. At postoperative 0, 5, 10, 15, 20 and 25 days, the rats were assigned to receive intra-muscular injection of 50 μl PBS (Group A, n = 30) and 10 μg/ml decorin ( Group B, n = 30) respectively according to the random number table. Skeletal muscle samples from the rats were removed 7, 14 and 28 days after the last injection. Scar of the skeletal muscle was observed by HE staining. Expressions of TGF-[31, p-Smad3 and collagen I were analysed by western blot. Results At days 14 and 28, HE staining showed tissue fibrosis in Group B was significantly reduced compared to Group A. At day 7, expression levels of TGF-β1, p-Smad3 and collagen I in Group A were O. 152 ±0. 031, 0. 439 ±0.035 and 0. 714 ±0. 116 respectively compared to 0. 145 ±0. 039, 0. 446 ± 0. 036 and 0. 712±0.078 in Group B (P 〉 0.05). At day 14, expression levels of TGF-β1, p-Smad3 and collagen [ in Group A were 0. 263 ± 0. 015, 1. 082 ±0. 017 and 1. 171 ± 0. 138 compared with 0. 136 ±0.004, 0. 155 ±0. 040 and 0. 217 ±0. 025 in Group B (P 〈0.05). At day 28, expression levels of TGF-β1, p-Smad3 and collagen I were 0. 754 ± 0. 042, 1. 197± 0. 086 and 1. 825 ± 0. 044 in Group A compared with 0. 307 ± 0. 006, 0. 193 ± 0. 028 and 0. 256± 0. 057 in Group B ( P 〈 0.05 ). Conclusion Local intra-muscular injection of decorin can reduce scar formation in a rat osteomyelitis model through inhibiting TGF-β-p-Smad3-collagen I signaling pathway.
