哌拉西林他唑巴坦皮试阴性后迟发型变态反应21例分析

摘　　要：目的了解哌拉西林他唑巴坦皮试阴性后出现的迟发型变态反应的特点和一般规律,为临床合理用药提供依据。方法回顾性分析我院住院患者21例哌拉西林他唑巴坦皮试阴性后出现的迟发型变态反应,从变态反应的发生率、临床表现、发生及缓解时间等多个方面进行统计和分析。结果哌拉西林他唑巴坦皮试阴性后迟发型变态反应的发生率不低于1‰~4‰,1~9岁年龄段患者迟发型变态反应发生率为1.16%,明显高于其他年龄段人群;迟发型变态反应出现时间越晚,患者变态反应的预后时间可能越长;β-内酰胺类有过敏史的患者出现迟发型变态反应的风险较高。结论哌拉西林他唑巴坦皮试阴性后出现的迟发型变态反应发生率较低,但其易发人群和变态反应发生时间等存在不确定性,医务人员应注意监测,确保临床用药的安全性。Objective To understand the characteristics and general rules of delayed type hypersensitivity by piperacillin/tazobactam with negative skin test,and to provide a reference for safe medication in clinic.Methods Inpatients in our hospital from July 1,2013 to June 30,2015,21 cases of delayed type hypersensitivity by piperacillin/tazobactam with negative skin test were retrospectively analyzed,in respect of statistical analysis of the incidence,symptoms,occurrence and remission of allergic reactions.Results The incidence rates of the delayed type hypersensitivity by piperacillin/tazobactam with negative skin test were at least more than1‰-4‰,the incidence rates of the 1-9years old was 1.16%,which is significantly higher than other age groups.The later time of the delayed type hypersensitivity reaction happened,the longer of the prognosis of the patients with allergic reaction occurred;the risk of delayed type hypersensitivity was higher in patients with allergic history.Conclusion Even although the incidence rates of the delayed type hypersensitivity by piperacillin/tazobactam with negative skin test was low,the prone group and allergic reaction time was uncertain.We need pay more attention for piperacillin/tazobactam use to ensure the safety of clinical medication.
