单病种分值付费制下五个病种住院费用的影响因素分析

摘　　要：目的:探讨中山市单病种分值付费制下五个病种的患者住院费用的影响因素,采取有效措施,控制住院费用,提高医院的经济效益和社会效益。材料与方法:调取中山市某医院病案统计管理系统2010年1月至2012年12月主要诊断前五位病种的患者资料,分别是Ⅱ型糖尿病伴多个并发症的1022例、高血压病3级的953例、肺部感染的2234例、脑梗塞的2060例和肿瘤化学治疗疗程的2284例,收集这五个病种的患者临床资料,包括住院费用、性别、年龄、住院天数、入院时情况、医疗保障、疾病合并症、医院感染、出院时情况等。利用SPSS13.0对上述资料进行线性回归分析。结果:年龄、住院天数、医疗保障、疾病合并症、医院感染是控制五个病种的患者住院费用的影响因素。结论:减少住院天数,合理应用医疗保障,避免医院感染和疾病合并症,有效控制住院费用。Purpose： To investigate the influential factors of the hospitalization expenses of five disease entities under Zhongshan disease score system of payment,took effective measures to reduce the economic pressure of patients and control the hospital costs. Materials and Methods： Transferedthe statistical datas of the top five primary diagnosis disease entities of medical record management system diseases from January 2010 to December 2012 in a hospital of Zhongshan City. Among those patients with Type Ⅱ diabetes with multiple complications were 1022 cases,grade 3 hypertension were 953 cases,pulmonary infection were 2234 cases,cerebral infarction were 2060 cases and cancer chemotherapeutic were 2284 cases. Collected the clinical datas of the five disease entities,including hospital charges,sex,age,length of stay,admission conditions,health care,disease complications,hospital infection,discharge,etc. Linear regression analyzedon the above information using SPSS16. 0. Results： Thefive factors of age,length of stay,medical insurance,disease complications,hospital infections were the influential factors of the hospitalization expenses of the five disease entities. Conclusion： Byencouraging regular physical examination,avoiding disease complications,preventing hospital infections,shortening hospital stay,strengthening medical insurance,reducing patients＇ economic pressure can effectively control the hospitalization expenses and solve the expensive problem.
