摘 要:目的分析新生儿呼吸机相关性肺炎的高危因素和病原学状况。方法 125例机械通气>48 h的新生儿,其中呼吸机相关性肺炎55例(A组),非呼吸机相关性肺炎70例(B组),分析呼吸机相关性肺炎的高危因素和病原菌。结果两组患儿胎龄、出生体重、呼吸机持续时间、气管切开、留置胃管比较差异具有统计学意义(P<0.05);Logistic回归分析显示呼吸机持续时间、气管切开、留置胃管均是新生儿呼吸机相关性肺炎的高危因素(P<0.05);A组细菌培养革兰阴性菌占72.4%。结论新生儿呼吸机相关性肺炎的临床高危因素多样,革兰阴性菌是主要致病菌。Objective To analyze high risk factor and etiology condition in neonatal ventilator-associated pneumonia. Methods Among 125 newborn with mechanical ventilation 48 h, there were 55 cases with ventilator-associated pneumonia(group A) and 70 cases without ventilator-associated pneumonia(group B). High risk factor and pathogenic bacteria of ventilator-associated pneumonia were analyzed. Results The difference of gestational age, birth weight, ventilator duration, tracheotomy, and indwelling gastric tube had statistical significance between the two groups(P0.05). Logistic regression analysis showed high risk factors of neonatal ventilator-associated pneumonia as ventilator duration, tracheotomy, and indwelling gastric tube(P0.05). Gramnegative bacteria accounted for 72.4% in group A bacterial culture. Conclusion Clinical high risk factor of neonatal ventilator-associated pneumonia is various, and gram-negative bacteria is the main pathogenic bacteria.
