早期糖尿病肾病患者尿清蛋白排泄率、血CysC及Hcy水平表达及意义

摘　　要：目的探讨早期糖尿病肾病患者尿清蛋白排泄率、血Cys C及Hcy水平表达及意义。方法选择150例2型糖尿病肾病患者作为研究对象,根据其尿白蛋白排泄率将其分为早期糖尿病肾病组(A组,50例)、单纯型糖尿病肾病组(B组,50例)和临床糖尿病肾病组(C组,50例),选择同期在我院进行健康体检者50例作为对照组研究对象(D组)。测定并比较不同组研究对象的血Cys C及Hcy水平,分析其与尿清蛋白排泄率之间的相关性。结果 3组2型糖尿病肾病患者的血Cys C及Hcy水平均显著高于对照组患者(P<0.05);其中A组患者血Cys C及Hcy水平显著高于B组患者(P<0.05);C组患者显著高于A组(P<0.05);三组患者的血Cys C及Hcy水平和尿清蛋白排泄率均呈现正相关关系(r=0.654,0.791,0.652)。结论血Cys C及Hcy可以作为敏感指标反映早期糖尿病肾病患者的肾损伤,随着糖尿病肾病的进展,二者水平随尿清蛋白排泄率的升高而升高。Objective To investigate the early diabetic nephropathy patients with urinary albumin excretion rate,and clinical significance of serum Cys C and the expression of Hcy. Methods 150 cases of type 2 diabetic nephropathy patients as the research object,according to the urine albumin excretion rate will be divided into the early diabetic nephropathy group（ 50 cases,group A）,diabetic DN group（ group B,50 cases） and clinical diabetic nephropathy group（ C group,50 cases）,select the same period in our hospital for health examination in 50 cases as control subjects（ group D）. To determine and compare the different groups of subjects of blood Cys C and Hcy levels,analyze the correlation between urinary albumin excretion rate. Results The 3 groups of patients with type 2 diabetic nephropathy of blood Cys C and Hcy levels were significantly higher than those in control group（ P 〈0. 05）; group A patients with serum Cys C and Hcy levels were significantly higher than those in group B（ P 〈 0. 05）; patients in group C were significantly higher than that of A group（ P 〈 0. 05）; three groups of patients of serum Cys C and Hcy levels and urinary albumin excretion rate showed a positive correlation（ r = 0. 654,0. 791,0. 652）. Conclusions The serum Cys C and Hcy can be used as sensitive indicators reflect renal damage in patients with early diabetic nephropathy,with the progression of diabetic nephropathy,second level with urinary albumin excretion rate increased and increased.
