慢性心力衰竭患者血miRNA-21的变化及意义

摘　　要：目的:探讨mi RNA-21在慢性心力衰竭患者中表达的差异及临床意义。方法:入选2014年11月至2015年12月在我科住院的慢性心力衰竭患者150例(CHF组)和体检中心无心力衰竭者45例(对照组)。根据NYHA心功能分级,CHF组被分为A组(心功能Ⅱ级,49例),B组(心功能Ⅲ级,51例)和C组(心功能Ⅳ级,50例)3个亚组;运用荧光实时定量PCR(q RT-PCR)方法检测mi RNA-21含量。测定B型脑钠肽(BNP)、左室射血分数(LVEF)及左室舒张末内径(LVEDD)等。结果:CHF各组患者mi RNA-21水平均较对照组高,并且C组较A、B组明显升高,B组较A组明显升高,差异有统计学意义(P<0.01)。相关分析表明:mi RNA-21的表达量与BNP(r=0.763,P<0.01)、LVEDD(r=0.691,P<0.01)呈正相关,与LVEF呈负相关(r=-0.918,P<0.01)。结论:mi RNA-21可能为心力衰竭潜在的诊断学标记物,并可为慢性心力衰竭诊断及预后评估提供重要参考依据。Objective To investigate the expression difference of micro RNA-21（mi RNA-21） in patients with chronic cardiac function and analyze its clinical significance. Methods Patients with chronic heart failure（trial group,150 cases） and the subjects without chronic heart failure（control group,45 cases） were enrolled.Patients with chronic heart failure were divided into three subgroups according to NYHA ： group A（heart function class Ⅱ,49 cases）,group B（classⅢ, 51 cases） and group C（class Ⅳ, 50 cases）. mi RNA-21 levels were detected by q RT-PCR method. The levels of B type natriuretic peptide urea（BNP）,left ventricular end diastole diameter（LVEDD） and left ventricular ejection fraction（LVEF） were determined. Results mi RNA-21 expression in patients with heart failure were higher than that of control group（P〈0.01）,and mi RNA-21 expression in group C was higher than that of group A.Correlation analysis showed that there was a positive correlation between expression of BNP（r = 0.763,P〈0.01）, LVEDD（r = 0.691,P〈0.01）, and the level of mi RNA-21 in patients with chronic heart failure.And a negative correlation between LVEF value and the level of mi RNA-21（r =-0.918,P〈0.01）. Conclusion mi RNA-21 might be a potential marker for diagnosis of heart failure and could be a basis for reference about prognosis evaluation.
