慢性鼻-鼻窦炎主客观评价的相关性分析

摘　　要：目的探讨慢性鼻-鼻窦炎患者的主客观评分之间的相关性。方法对126例慢性鼻-鼻窦炎的患者进行主观评分(视觉模拟评估量表VAS,鼻腔-鼻窦结果测试SNOT-20)及客观检查评分(Lund-Mackay鼻窦CT影像与Lund-Kennedy内镜评分),采用Origin 8.0统计学软件包进行Pearson相关分析。结果主观评价VAS总分与SNOT-20总分之间呈显著相关(R2=0.482,P<0.01)。客观评价方式鼻内镜LK评分和Lund-Mackay CT评分之间呈高度相关(R2=0.59,P<0.01)。主观评价VAS、SNOT-20与客观评价方式鼻内镜LK评分无显著相关(P>0.05);VAS、SNOT-20与Lund-Mackay CT评分也无显著相关(P>0.05)。VAS评价中鼻塞、面部胀痛、嗅觉障碍、头痛单项症状评分与鼻内镜LK评分、Lund-Mackay CT评分间呈显著相关(P<0.01)。结论慢性鼻-鼻窦炎患者的主观评价与客观评价之间存在部分分离。故而,慢性鼻-鼻窦炎患者的评价需要结合主观症状和客观检查。Objective To explore the correlation between subjective and objective scoring in chronic rhino sinusitis. Methods Prospective data of 126 patients with CRS were collected and analyzed retrospectively. Subjective rhino sinusitis examinations include visual analog scale and the Sino nasal Outcomes Test-20. Objective sinus examinations include Lund-Kennedy nasal endoscopy score and Lund-Mackay computed tomography （CT） score. Pearson correlation analysis was performed by Origin 8.0 statistical package. Results There was significant correlation between VAS score and SNOT-20 score （R2=0.482, P〈0.01）. There was significantly correlation between Lund-Kennedy nasal endoscopy score and Lurid-Mackay CT score （R2=0.59, P〈0.01）. VAS score and SNOT-20 scores had no correlation with Lund- Kennedy nasal endoscopy score（P〉O.05）. VAS score and SNOT-20 scores had no correlation with Lund-Mackay CT score（P〉0.05）. There were significant correlation between Lund-Kennedy nasal endoscopy score and individual scores of VAS nasal obstruction, facial pain, dysosmia dysfunction and headache（P〈0.01）. Conclusion The subjective and objective score of chronic rhino sinusitis are contradictive sometimes. Therefore, the evaluation of CRS requires a combination of subjective symptoms and objective examination.
