机械通气危重症患儿血清白蛋白水平对临床预后的影响和临床意义研究

摘　　要：目的：探讨研究机械通气危重症患儿血清白蛋白水平对其临床预后的影响和临床意义。方法：回顾性分析2013-05-2014-06我院收治的机械通气危重症患儿96例,根据其血清白蛋白水平分为〈25g/L组、25~35g/L组和〉35g/L组,比较3组患儿的小儿危重病例评分（PICS）、多器官功能障碍综合征（MODS）受累脏器数目、机械通气时间、重症监护室入住时间、住院时间及放弃或死亡例数差异情况,并对PICS与血清白蛋白水平行Pearson相关分析。结果：患儿血清白蛋白水平与PICS呈显著正相关（r=0.352,P〈0.01）,且3组患者中,血清白蛋白越高者其MODS受累脏器数目越少,机械通气时间、重症监护室入住时间、住院时间越短,放弃或死亡例数也越少（P〈0.01）。结论：机械通气危重症患儿的血清白蛋白水平与其预后有着明显关联,血清白蛋白水平越低的患儿其预后越差,在临床上检测患儿血清白蛋白水平有利于评估其病情及预后情况。Objective：To study the clinical significance and prognostic effect of serum albumin in critically ill children with mechanical ventilation.Method：The clinical data of 96 critically ill children with mechanical ventilation who treated in our hospital from May 2013 to June 2014 were retrospective analyzed.According to their serum protein levels,all the patients were divided into25g/L group,25-35g/L group,35g/L group.Pediatric critical score,number of the failure organs,the duration of using mechanical ventilation,length of stay in the ICU,the time in hospital and cases of death or abandoning treatment were compared within three groups.Pearson＇s correlation was used in analyzing the relation of pediatric critical score and serum albumin levels.Result：There was a positive correlation between the serum albumin level and pediatric critical scores（r=0.352,P〈0.01）.The comparing analysis between three groups show that the higher levels of serum albumin,the less failure organs,the shorter time of using mechanical ventilation,stay in the ICU and hospital and the lower rate of death and abandoning treatment（P〈0.01）.Conclusion：There is a significantly correlation between the serum albumin levels and the prognosis in critically ill children with mechanical ventilation.The children with lower serum albumin levels who treated by mechanical ventilation have worse prognosis.The serum protein levels have predictive value in disease evolution and prognosis.
