2型糖尿病伴急性多发性脑梗死患者焦虑、抑郁情绪与认知功能障碍的相关性

摘　　要：目的研究2型糖尿病伴急性多发性脑梗死患者焦虑、抑郁情绪与认知功能障碍的相关性。方法 2型糖尿病患者69例,根据头颅MRI影像学检查结果分为2型糖尿病伴急性多发性脑梗死组(研究组)34例,平均年龄(66.53±9.25)岁;不伴脑梗死的单纯2型糖尿病组(对照组)35例,平均年龄(62.34±9.77)岁。受试者均测定身高、体重、血压、血脂谱、糖化血红蛋白(Hb A1c),采用汉密尔顿焦虑(HAMA)、抑郁(HAMD)量表评定两组焦虑、抑郁状况,采用蒙特利尔认知评估(Mo CA)量表测评两组认知功能。结果研究组收缩压及HAMA、HAMD评分均显著高于对照组(P<0.05)。研究组Mo CA评分显著低于对照组HAMD(P<0.001)。相关分析显示,研究组HAMD评分呈显著正相关(P<0.001),HAMD评分与Mo CA评分呈负相关(P<0.05)。结论 2型糖尿病伴急性多发性脑梗死患者比单纯糖尿病患者存在更多的焦虑、抑郁情绪及认知功能障碍,焦虑、抑郁情绪常同时存在,抑郁情绪越明显,认知功能障碍越严重。Objective To explore the correlation of anxiety,depression and cognitive impairment in type 2 diabetic patients（T2DM） with acute multiple cerebral infarction.Methods 69 T2DM patients were included,34 T2DM patients with acute multiple cerebral infarction（study group）,and 35 T2DM patients without ischemic stroke（control group） were tested height,weight,blood pressure,lipid profile and glycosylatedhemoglobin.Anxiety,depression states were assessed by Hamilton Anxiety Scale（HAMA） and Hamilton depression scale（HAMD）,cognitive impairment with Montreal Cognitive Assessment（MoCA）.The data between the two groups were analyzed.Results The systolic blood pressure was higher in study group than that in control group（P=0.01）,the scores of the HAMA and HAMD were significantly higher than those in control group（P〈0.001）,the score of MoCA was significantly lower than that of control group（P〈0.001）.In study group,the score of HAMA was positively correlated with HAMD score（P〈0.001）,while HAMD score was negatively correlated with score of Mo CA（P〈0.05）.Conclusions Anxiety could lead to high blood pressure,and the more obvious depression is,the more serious cognitive dysfunction is.
