超声心动图诊断胎儿双主动脉弓的临床价值

摘　　要：目的探讨超声心动图诊断胎儿双主动脉弓(DAA)的临床价值。方法回顾性分析8例胎儿DAA的超声图像特征,并随访临床结果。结果 8例DAA(双侧弓通畅型6例,一侧弓闭锁型2例),5例属于单纯性DAA,其余3例(37.5%,3/8)并发其他心内外畸形,包括1例染色体异常。6例DAA经超声心动图筛出,引产终止妊娠,其中3例经尸检证实为DAA;1例误诊为右位主动脉弓(RAA)伴左位动脉导管,另1例误诊为RAA伴右位动脉导管,产后经超声心动图确诊为DAA,手术干预后的疗效满意。结论超声心动图是诊断胎儿DAA的可靠方法,早期明确诊断有利于胎儿的预后评估。Objective To explore the value of echocardiography in diagnosis of fetal double aortic arch（DAA）.Methods A retrospective analysis was made on the ultrasonographical features of 8cases with DAA and then the clinical results were followed-up.Results There were 8cases of DAA fetus（including：6cases with bilateral aortic arch patency type,2cases with unilateral arch blocking type）.5cases were isolated DAA,the other 3cases（37.5%,3/8）were complicated with other internal and external cardiac malformations,including one case of abnormal chromosome.6cases of DAA were diagnosed by prenatal echocardiography and the termination of pregnancy was selected in all cases by the selection of labor,and 3cases were confirmed by autopsy as DAA.One case was misdiagnosed as right aortic arch（RAA）with left duct arteriosus and the other one misdiagnosed as RAA with right duct artefiosus.Postpartum echocardiography confirmed as the DAA and the curative effect was satisfied after surgical treatment for these two cases.Conclusions Echocardiography is a reliable method for the prenatal diagnosis of DAA and early diagnosis is beneficial to the evaluation of the prognosis of the fetus.
