上海社区缺血性脑卒中二级精准预防服药情况分析

摘　　要：目的调查上海社区缺血性脑卒中患者二级预防服药依从性及服药现状。方法回顾性分析上海社区500例缺血性脑卒中患者服药情况,并通过电话或门诊随访形式对入选患者12个月内服药及停药情况实施调查。结果 (1)缺血性脑卒中二级预防服药依从性主要影响因素为受教育程度、是否医保报销和卒中亚型。患者出院后服用阿司匹林各时间点依从性比较,P<0.05。患者出院6个月、12个月时服用阿司匹林依从性明显低于出院1个月时,P<0.05。(2)患者出院后12个月时服用四种二级预防药物的持续性比较,P<0.05;服用阿司匹林的服药持续性明显高于双联及氯吡格雷,P<0.05。结论缺血性脑卒中患者出院后二级预防药物服用依从性较差,且受多种因素影响,因此,医院需加强对患者二级预防药物服用的健康教育及正确指引。OBJECTIVE Shanghai Community survey was conducted on the secondary prevention and medication status quo of isch-emic stroke among patients with medication compliance. METHODS A retrospective analysis was conducted on 500 cases of ShanghaiCommunity ischemic stroke patients taking the case. Patients taking the case within 12 months and the withdrawal were investigatedby telephone or outpatient follow-up form. RESULTS（1）Main factors for secondary prevention of ischemic stroke drug compliance in-cluding the level of education, whether had the Medicare reimbursement, stroke subtype. After the patient was discharged aspirin com-pliance at each time point comparison, P〈0.05. The patient was discharged within six months, aspirin compliance was significantlylower than its discharge within one month, P〈0.05 12 months.（2） Patients discharged from hospital after taking 12 months continuouscomparison of four kinds of secondary prevention drugs, P〈0.05; then continue taking aspirin was significantly higher than double andclopidogrel, P〈0.05. CONCLUSION After discharge, patients with ischemic stroke secondary prevention of drug medication compli-ance were poor, and affected by many factors, so hospitals needed to strengthen health education and guidance on the proper medica-tion for patients under secondary prevention with medication.
