益气养阴活血汤治疗气阴两虚型早期糖尿病肾病临床疗效研究

摘　　要：目的探讨益气养阴活血中药治疗气阴两虚型早期糖尿病肾病(DN)的临床疗效。方法选择2014年12月至2015年12月医院收治且辨证为气阴两虚型DN患者110例,采用随机、单盲法分作两组,其中对照组55例采取常规西药治疗,观察组55例加用益气养阴活血汤,对比两组患者临床疗效及治疗前后血糖、血脂水平和肾功能、血液流变学指标变化。结果观察组总有效率为87.27%,较对照组的65.45%明显提高(P<0.05)。治疗后,观察组患者治疗后空腹血糖(FBG)为(6.67±0.56)mmol/L,总胆固醇(TC)为(3.78±0.49)mmol/L,甘油三酯(TG)为(1.43±0.34)mmol/L,血清肌酐(SCr)为(70.61±6.49)μmol/L,尿素氮(BUN)为(4.71±0.44)mmol/L,尿微量白蛋白(UAER)为(32.22±4.12)mg/24 h,与治疗前和对照组治疗后比较均有明显减少(P<0.05)。观察组患者全血低切黏度为(7.85±0.79)mPa·s,全血高切黏度为(4.32±0.49)mPa·s,血浆黏度为(1.46±0.15)mPa·s,纤维蛋白原为(3.24±0.45)g/L,红细胞压积为(38.48±4.03)%,与治疗前和对照组比较均有明显减少(P<0.05)。结论益气养阴活血汤治疗气阴两虚型DN疗效显著,可改善肾脏微循环,保护肾脏功能,延缓疾病进展,值得临床推广。Objective To explore the clinical efficacy of Yiqi Yangyin Huoxue Decoction in treating diabetic nephropathy with Qiyin Deficiency. Methods 110 patients with Qiyin Deficiency type of diabetic nephropathy from December 2014 to December 2015 were divided into two groups by randomized single- blind method, 55 cases in each group. The control group was given conventional medicine treatment, while the observation group was given Yiqi Yangyin Huoxue Decoction on the basis of the control group. The blood glucose, blood lipid levels and kidney function, blood rheology changes were compared between the two groups before and after treatment. Results The total effective rate of the observation group was 87.27%, which was significantly higher than 65.45% of the control group（ P 〈 0. 05）. After treatment, the FBG of the observation gorup was （6. 67±0. 56）mmol/L, TC was （3.78 ±0.49）mmol/L, TG was （1.43 ±0.34）mmol/L, SCr was （70.61±6.49）μmol/L, BUN was （4.71±0.44）mmol/L, UAER was （32.22±4.12）mg/24 h, which had significantly decreased compared with pre- treatment and the control group（ P 〈 0. 05）. After treatment, the low shear whole blood viscosity was （7.85 ± 0. 79 ） mPa · s, high shear whole blood viscosity was （4.32 ±0. 49）mPa ·s, plasma viscosity were （ 1.46 ±0. 15 ）mPa·s, fibrinogen were （3.24 ±0. 45）g/L, hematocrit was （38.48±4. 03）% in the observation group, which had significantly decreased compared with pre- treatment and the control group （P 〈 0.05）. Conclusion Yiqi Yangyin Huoxue Decoction in treating Qiyin Deficiency type of diabetic nephropathy has significant effect; it can improve renal microcirculation, protect the kidney function, and delay disease pro-gression, which is worthy of clinical promotion.
