摘　　要：目的探讨结核感染T细胞斑点试验(T-SPOT.TB)诊断乳腺结核的临床价值。方法 13例经组织病理学检查证实的单纯乳腺结核患者(乳腺结核组),10例体检健康者为对照组,均行T-SPOT.TB和结核菌素皮肤试验(tuberculin skin test,TST)检测。与组织病理学检查结果进行对照,比较2种方法诊断乳腺结核的敏感性和特异性。结果乳腺结核组T-SPOT.TB检测11例阳性、2例阴性,TST检测7例阳性、6例阴性;对照组T-SPOT.TB检测2例阳性、8例阴性,TST检测4例阳性、6例阴性;T-SPOT.TB诊断乳腺结核的敏感性(84.6%)高于TST(53.8%)(P<0.05),特异性(80.0%)与TST(60.0%)比较差异无统计学意义(P>0.05)。结论 T-SPOT.TB是诊断乳腺结核的敏感性较高的辅助方法,对乳腺结核的早期诊断有重要价值。Objective To assess the value of T-SPOT. TB to the diagnosis of breast tuberculosis （BTB）. Methods Thirteen patients with primary BTB （BTB group） and 10 healthy volunteers （control group） received T-SPOT. TB and tuberculin skin test （TST）, and were compared the sensitivity and specificity of T-SPOT. TB and TST with the histopathological results. Results T-SPOT. TB result showed positive in 11 patients and negative in 2 patients, and TST result showed positive in 7 patients and negative in 6 patients in BTB group. T-SPOT. TB result showed positive in 2 and negative in 8, and TST result showed positive in 4 and negative in 6 in control group. The sensitivity of T-SPOT. TB test for BTB （84.6 % ） was significantly higher than that of TST （53.8 % ） （P〈0.05）, and there was no significant difference in the specificity between T SPOT. TB （80.0%） and TST （60.0%） for BTB （P〉0.05）. Conclusion T-SPOT. TB is a useful adjunct test for the diagnosis of BTB with a high sensitivity, and has a great value to the early diagnosis of BTB.
