摘  要:目的检测血清超敏C反应蛋白(hs-CRP)、脂蛋白相关磷脂酶A_2(LP-PLA_2)和类胰蛋白酶(TPS)在高血压患者中的变化,分析它们与高血压合并冠心病的相关性,探讨3个指标在高血压伴冠心病中的临床意义。方法选取符合纳入标准的高血压患者155例,按照是否合并冠心病分为单纯高血压组75例、高血压合并冠心病组80例,选择健康体检者48例作为对照组。采用ELISA法检测LP-PLA2、TPS水平,比较各组间差异。应用Logistic回归分析高血压合并冠心病的危险因素和炎性指标。利用ROC曲线分析hs-CRP、LP-PLA_2、TPS诊断冠心病的临界点,分析其对高血压合并冠心病的预测价值。结果高血压合并冠心病组hs-CRP、LP-PLA_2、TPS水平较单纯高血压组、对照组明显升高(P<0.05)。Logistic回归结果显示吸烟史、hs-CRP、LP-PLA_2、TPS是高血压合并冠心病的危险因素(P<0.05),hs-CRP、LP-PLA_2、TPS三者ROC曲线下面积分别为0.775、0.656、0.793。结论hs-CRP、LP-PLA_2、TPS与高血压合并冠心病有一定的相关性。hs-CRP、LP-PLA2、TPS是高血压合并冠心病的危险因素,LP-PLA_2、TPS对高血压合并冠心病的预测与hs-CRP相比诊断价值相当,可作为高血压合并冠心病患者临床指标,指导其临床诊疗和预后评估。Objective To detect the serum levels of hypersensitive C-reactive protein ( hs-CRP), lipoprotein associated phospholipase A2 (LP-PLA2 )and tryptase(TPS) in patients with hy-pertension. To analyze the correlation of them and hypertension with coronary heart disease, and to explore clinical significance of the three leukocytes in the hypertension with coronary heart disease. Methods A total of 155 patients were participated in the study, all the patients were divided into three groups, pure hypertension group of 75 cases, hypertension with coronary heart disease group of 80 cases, and 48 normal controls enrolled in the study. Using ELISA detect the level of LP-PLA2 , TPS. The differences of the two leukocytes levels between three groups were analyzed. The risk factors of hypertension with coronary heart disease were analyzed with logistic regression analysis, and ROC curve we-re used to evaluate the diagnostic value of hs-CRP, LP-PLA2 and TPS for coronary heart disease. Results The concentrations of hs-CRP, LP-PLA2 and TPS in the hypertension with coronary heart disease group were higher than that in the pure hypertension group and the normal controls( P 〈0.05 ). Logistic regression analysis demonstrated that hs-CRP, LP-PLA2, and TPS are the risk factors of hypertension with coronary heart disease ( P 〈 0.05 ). The area under the ROC curve were 0. 775,0. 656,0. 793. Conclusion The serum levels of hs-CRP, LP- PLA2 and TPS have a certain correlation with hypertension with coronary heart dis-ease, hs-CRP, LP-PLA2, and TPS are the risk factors of hypertension with coronary heart disease. They could be important forecast factors in the clinical evaluation of hypertension with coronary heart disease.
