摘  要:目的探讨剪切波弹性成像(SWE)评估不同回声颈动脉斑块弹性特征的价值。方法对145例颈动脉斑块患者行颈动脉超声及SWE检查,采用两种斑块回声分类方法对斑块进行分类,比较不同回声特征颈动脉斑块的杨氏模量值。结果低回声、等回声、强回声及不均质回声最大杨氏模量值、平均杨氏模量值比较差异均有统计学意义(均P〈0.05),两两比较差异均有统计学意义(均P〈0.05);最小杨氏模量值在四种回声斑块中的差异均有统计学意义(均P〈0.05),两两比较除低回声与等回声之间差异无统计学意义外,其余差异均有统计学意义(均P〈0.05)。Ⅰ~Ⅳ型斑块最大杨氏模量值、平均杨氏模量值比较差异均有统计学意义(均P〈0.05),两两比较除Ⅲ型和Ⅳ型斑块之间差异无统计学意义外,其余差异均有统计学意义(均P〈0.05);四种回声斑块中最小杨氏模量值比较差异均有统计学意义(均P〈0.05),两两比较除Ⅰ型和Ⅱ型斑块之间差异无统计学意义外,其余差异均有统计学意义(均P〈0.05)。结论 SWE技术可以定量评估颈动脉斑块的弹性特征,能够反映不同回声斑块间的弹性差异。Objective To explore the application value of shear wave elastography(SWE)in evaluating the elastic characteristics of carotid plaque with different echogenicity.Methods One hundred and forty-five patients were detected by carotid artery ultrasonography,all the carotid plaques underwent SWE and were classified by two kinds of plaque echogenicity classification methods. The Young's modulus values of carotid plaques with different echogenicity were compared. Results There were statistical differences in maximum and mean Young's modulus values of hyperechoic,hypoechoic,isoechoic and hetrogeneous carotid plaques(all P〈0.05).Minimum Young's modulus value in the four types of carotid plaques were statistically different(all P〈0.05).The difference between hypoechoic and isoechoic carotid plaques was not significant,the other differences between two interval members were statistical(P〈0.05).The average young's modulus value among the four types of plaques were were statistically different(all P〈0.05).The differences between the two interval members were statistically different(P〈0.05). There were significant differences of maximum and mean modulus values among Ⅰ~ Ⅳ type carotid plaques(all P〈0.05). The difference between the type Ⅲ and type Ⅳ plaques was not significant,the other differences were statisticallydifferent(P〈0.05).The minimum Young's modulus value in the four types of plaque were statistically different(all P〈0. 05). The difference between type Ⅰ and type Ⅱ plaques was not significant,the other differences between the two interval members were statistically different(P〈0.05). Conclusion SWE can quantitatively evaluate the elastic characteristics of carotid plaque and reflect the elastic variances of plaques with different echoes.
