感染性休克患者采用丙泊酚麻醉镇静对容量反应性的影响研究

摘　　要：目的探讨感染性休克患者应用丙泊酚麻醉镇静对容量反应性的影响,给临床诊断提供理论参考依据。方法选取2014年8月-2015年8月在医院接受治疗早期目标治疗(EGDT)时,液体复苏达到标准后短期内出现感染性休克的50例患者作为研究对象,随机分成两组,对照组25例给予咪达唑仑进行麻醉镇静,观察组患者25例给予丙泊酚进行镇静麻醉作用;比较两组患者的容量反应性的变化和容量负荷。结果观察组在镇静后容量反应性增加的比例明显高于对照组,差异有统计学意义(P<0.05);观察组镇静后容量有反应性亚型患者的比例增加66.7%,增加的幅度明显高于对照组的27.3%。结论丙泊酚应用感染性休克患者的麻醉和镇静,对容量反应性有增加的趋势,不会影响心肌收缩力或是影响很小,值得临床推广使用。OBJECTIVE To investigate effect on capacity reactivity of using propofol anesthetic sedative in patients with septic shock,so as to provide a theoretical basis for clinical diagnosis.METHODS A total of 50 patients got septic shock after the fluid resuscitation reached standard in our hospital for treatment of early treatment（EGDT）in Aug.2014 to Aug.2015 were selected as research subjects,who were randomly divided into two groups,the control group with 25 patients given midazolam sedation,observation group 25 patients given propofol for anesthetic sedation.The change and volume load the capacity load of capacity reactivity between the two groups of patients were compared.RESULTS The proportion of capacity reactivity increase after sedation in observation group were significantly higher than that in control group（P〈0.05）;the proportion of subgroups of patients with reactive for capacity after sedation in observation group increased 66.7%,which was significantly higher than that of the control group（27.3%）.CONCLUSION Using propofol for anesthesia and sedation in patients with infectious shock could give a increasing trend for volume responsiveness,and will not affect myocardial contractility or just little,so which is worthy of clinical use.
