糖尿病下肢动脉病变合并幽门螺杆菌感染患者炎症因子和血管内皮功能变化研究

摘　　要：目的探讨糖尿病下肢动脉病变(PAD)合并幽门螺杆菌(Hp)感染患者介入治疗前后炎症因子、血管内皮功能变化,为糖尿病PAD的治疗提供参考。方法选取2015年1-8月住院治疗120例患者,按疾病分为PAD组与对照组,各60例,比较两组患者Hp感染率及炎症因子,数据采用SPSS 19.0软件进行统计分析。结果 Hp阳性者PAD组37例、对照组22例,感染率分别为61.67%、36.67%;入院时高敏C-反应蛋白(hs-CRP)、血管性血友病因子(vWF)、内皮素(ET)水平PAD组均高于对照组,两组比较,差异均有统计学意义(P<0.05),hsCRP、vWF、ET水平PAD组术后24h时高于入院时。结论糖尿病下肢动脉病变患者与单纯糖尿病比较Hp感染率更高,炎症反应及内皮功能紊乱更明显,介入手术可加重内皮功能损伤及炎症反应。OBJECTIVE To analyze Helicobacter pylori（Hp）infection status and inflammatory factors and endothelial function changes before and after interventional therapy in patients with diabetes peripheral arterial diseases（PAD）,so as to provide references for the treatment of diabetic PAD.METHODS A total of 120 cases of patients in hospital were selected from Jan.to Aug.2015,and divided into PAD group and control group according to the disease,with each group of 60 cases.The Hp infection rate and inflammatory factors of the two groups were compared.The data were analyzed by SPSS 19.0.RESULTS of There were 37 cases in PAD group and 22 cases in control group of Hp positive,with the infection rates of 61.67% and 36.67%,and the difference was significant（P〈0.05）.The hs-CRP,vWF and ET levels of patients in PAD group were higher than those in control group at admission（P〈0.05）.The hs-CRP,vWF,ET level in 24 hafter operation was higher than at admission in patients of PAD group.CONCLUSIONDiabetic PAD patients have higher infection rate of HP than simple diabetes,their inflammation and endothelial dysfunction are more obvious,and intervention surgery can aggravate the damage of endothelial function and inflammatory response.
