健脾益肾法联合中药结肠灌洗治疗糖尿病肾病37例  

Invigorating Spleen combined with Benefiting Kidney and Chinese Medicine Colon Lavage Method in the Treatment of Diabetic Nephropathy for 37 Cases

摘　　要：目的观察健脾益肾法联合中药结肠灌洗治疗糖尿病肾病(DN)的临床疗效。方法将73例糖尿病肾病患者随机分为两组,两组患者均给予糖尿病肾病常规治疗,优质低蛋白饮食,适当运动,皮下注射胰岛素控制血糖,严格控制血压。治疗组37例,给予健脾益肾中药汤剂口服同时联合中药汤剂结肠灌洗治疗。对照组36例,予厄贝沙坦治疗。两组均2个月为一疗程,共治疗3个疗程。监测治疗前后分别测定空腹血糖(FPG)、餐后2 h血糖(2h PG)、24 h尿蛋白、尿白蛋白排泄率(UAER)、尿素氮(BUN)、血清肌酐(Scr),统计临床疗效。结果治疗组总有效率86.49%,对照组总有效率66.67%,2组总有效率比较差异有统计学意义(P<0.05)。治疗组治疗后Scr、BUN与治疗前比较均明显降低,差异有统计学意义(P<0.05),且低于对照组治疗后(P<0.05)。结论健脾益肾法联合中药结肠灌洗能显著改善糖尿病肾病患者肾功能。Objective To observe the clinical effect of invigorating spleen and benefiting kidney and Chinese medicine colon lavage method in treating diabetic nephropathy. Methods 73 cases of diabetic nephropathy were randomly divided into two groups. Two groups of patients were given conventional treatment of diabetic nephropathy, high quality low protein diet, exercise, blood glucose control and subcutaneous injection of insulin, strict control of blood pressure. 37 cases in the treatment group were given Jianpi Yishen decoction combined with TCM decoction and colon lavage. The control group of 36 cases was treated with irbesartan treatment. The two groups were treated for 2 months which was a course of treatment, and there were a total of 3 courses of treatment.Determination of fasting blood glucose levels were monitored before and after treatment（FPG）, 2h postprandial blood glucose（2h PG）,24 h urinary protein, urinary albumin excretion rate（UAER）, urea nitrogen（BUN）, serum creatinine（Scr）, and the clinical effect was observed. Results The total effective rate of the treatment group was 86.49%, total effective rate of the control group was 66.67%,and the total effective rate of the two groups was statistically significant（P〈0.05）. After treatment, the BUN and Scr of the treatment group were significantly reduced compared with before treatment, and the difference was statistically significant（P〈0.05）, and which was lower than that of the control group after treatment（P〈0.05）. Conclusion The kidney function of the patients with diabetic nephropathy can be improved by the invigorating spleen and benefiting kidney and Chinese medicine colon lavage method.
