阿坝地区大骨节病患者尿元素谱及生化指标分析

摘  要:目的比较阿坝地区大骨节患者与健康对照人群尿元素谱及生化指标差异。方法采用分层整群抽样选择阿坝地区242名大骨节病患者以及79名健康对照,测定人群尿中18种元素浓度和吡啶酚、羟脯氨酸、肌酐水平。结果与对照组比较,病例组人群尿中微量元素Se、I的浓度较低,微量元素V、Cr、Mn的水平较高,差异均有统计学意义(P<0.05);而两组人群尿中常量元素(Co、Cu、Sr、Mo、Cd、Ca、Fe、K、Mg、Na、P、S、Zn)水平间比较,差异均无统计学意义(P>0.05)。病例组人群尿中吡啶酚的水平高于对照组,差异有统计学意义(P<0.01);而两组人群尿中羟脯氨酸的水平间比较,差异无统计学意义(P>0.05)。尿中V、Cr、Mn、吡啶酚与大骨节病病情程度呈统计学正相关(P<0.01)。结论阿坝地区大骨节病患者与对照人群尿中多种元素水平存在一定差异。Objective To know the difference of multi-element levels and biochemical indexes in the urine of patients with Kashin-Beck disease(KBD) and the healthy control group in parts of Aba Autonomous prefecture,Sichuan province. Methods A stratified cluster sampling was performed to select 242 KBD patients and 79 healthy controls,the concentrations of 18 kinds of elements, pyridinoline, hydroxyproline and creatinine were determined. Results Compared with the control group,the concentration of Se,I in urine of KBD patients were lower,the concentration of V,Cr,Mn were higher,the differences were significant(P〈0.05),and there was no significant difference between two groups in the concentration of other elements in urine.The concentration of pyridinoline in urine of KBD patients was significant higher than that in the control group, while the difference between two groups in the concentration of hydroxyproline in urine was not significant. There was significant positive correlation between the degrees of KBD and the concentration of V,Cr,Mn as well as the value of urine pyridine phenol and urine creatinine. Conclusion There is significant concentration difference of the elements in urine between KBD patients and the healthy control group.
