两性霉素B脂质体治疗侵袭性真菌感染的疗效观察

摘　　要：目的:探讨两性霉素B脂质体治疗重症监护病房(intensive care unit,ICU)侵袭性真菌感染(invasive fungal innfections,IFI)的临床疗效及安全性。方法:观察35例急救科ICU IFI患者使用两性霉素B脂质体的临床疗效及不良反应发生情况。结果:35例患者中,痊愈7例,显效18例,进步5例,无效5例,总有效率为71.43%,其中,白色假丝酵母菌和近平滑假丝酵母菌感染患者的治疗总有效率分别为77.78%(7/9)和83.33%(5/6);患者共发生不良反应16例,均较轻微,经对症治疗后均能得到控制。结论:两性霉素B脂质体用于ICU IFI患者的疗效确切,不良反应较少,在临床应用上有较大的优势。OBJECTIVE： To probe into the clinical efficacy and safety of amphotericin B liposome in treatment of invasive fungal infections（ IFI） in intensive care unit（ ICU）. METHODS： The clinical efficacy and adverse drug reactions of amphotericin B liposome in treatment of 35 patients with IFI in ICU from emergency department were observed. RESULTS：Of the 35 cases,there were 7 cases cured,18 cases with marked effects,5 cases with progress and 5 cases inefficacy. The total effective rate was 71. 43%,among which,the total effective rates of candida albicans and candida parapsilosis were respectively 77. 78%（ 7 /9） and 83. 33%（ 5 /6）. There were 16 cases of mild adverse drug reactions,which could be controlled after symptomatic treatment. CONCLUSIONS： The efficacy of amphotericin B liposome in treatment of IFI in ICU is significant,with few adverse drug reactions. It possesses great advantage in clinical application.
