老年冠心病患者外周血细胞角蛋白水平改变及其辅助诊断价值

摘　　要：目的检测老年冠心病患者外周血细胞角蛋白8(CK8)水平的改变,并分析其辅助诊断价值。方法纳入2013年1月至2015年1月收治的冠心病患者,并设定为观察组。另将同期接受体检者设定为对照组。检测两组外周血CK8水平并分析临床意义。结果本研究纳入96例研究对象,观察组46例,对照组50例。观察组患者外周血C反应蛋白(CRP)、白细胞介素6(IL-6)及CK8水平显著高于对照组(P<0.05)。观察组患者外周血CK8水平与CRP及IL-6均呈现显著正相关关系(P<0.05)。受试者工作曲线分析显示,以45.05μg/L为截点值,CK8诊断冠心病的敏感度为91.30%,特异度为92.00%,优于CRP及IL-6。观察组患者外周血CK8水平与Gensini评分呈现显著正相关关系(P<0.001)。结论 CK8在老年冠心病患者外周血表达升高且与疾病严重程度相关,具有一定辅助诊断价值。Objective To analyze the expression of serum Cytokeratin 8（CK8）in elderly patients with coronary heart disease and its auxilia-ry diagnostic value. Methods Clinical data of patients with coronary heart disease received treatment at our hospital from 2012 to 2015 were analyzed as Group A. Healthy cases were enrolled as Group B. Results A total of 96 cases were analyzed,Group A 46 cases,Group B 50 cases. Group A patients had higher levels of CK8,CRP and IL - 6 than these of Group B,the difference were statistically significant（ P ﹤ 0. 05）. Correlation analysis showed that,for Group A patients,the levels of CK8 had significant positive correlation with CRP and IL - 6（ P ﹤ 0. 05）. ROC analysis showed that the CK8 cut - off point value of 45. 05 μg/ L,as used for the diagnosis of coronary heart disease,provided a sensitivity of 91. 30% , a specificity of 92. 00% ,better than these of CRP and IL - 6. For Group A patients,the levels of CK8 had a significant positive correlation with Gensini scores（ P ﹤ 0. 001）. Conclusion The levels of CK8 increase significantly in elderly patients with coronary heart disease and show a posi-tive correlation with disease severity,these indicate that CK8 can be used as an auxiliary diagnostic marker for coronary heart disease.
