体外冲击波联合关节松动术对肩周炎的治疗效果研究

摘　　要：目的探讨体外冲击波联合关节松动术对肩周炎的治疗效果,为临床治疗肩周炎患者的方法选择提供参考依据。方法选取2014年8月至2015年间8月期间收治的肩周炎患者80例,按数字随机分为观察组40例和对照组40例。所有患者均接受常规肩周炎康复治疗,观察组采用体外冲击波与关节松动术相结合的方法进行治疗,对照组仅采用关节松动术进行治疗。比较两组患者的治疗有效率、患者治疗前、后肩关节功能的改变情况以及患者的疼痛程度视觉模拟评分(VAS)等指标的差异,对体外冲击波联合关节松动术的治疗效果进行评价。结果观察组患者的治疗总有效率明显高于对照组,差异有统计学意义(P<0.05)。治疗后观察组肩功能日常生活能力评分(ADL评分)和关节活动度(ROM)评分以及总评分明显高于对照组,差异有统计学意义(P<0.05)。治疗后观察组疼痛程度VAS评分明显低于对照组,差异有统计学意义(P<0.05)。结论使用体外冲击波联合关节松动术的治疗方法能明显提高肩周炎患者的临床疗效,改善患者肩功能,减小患者疼痛感,是优于单纯关节松动术的治疗方法,建议临床推广应用。Objectiye To analyse the effect of extracorporeal shock wave and manipulation treatment in scapulohumeral periarthritis and to provide a reference basis for clinical utility. Methods During August 2014 to August 2015,80 patients with scapulohumeral periarthritis were selected,who were divided into observation group and control group,40 ptients in each group. All patients were accepted conventional periarthritis of shoulder rehabilitation therapy,observation group adopted the combination of extracorporeal shock wave and joint mobilization treatment method, the control group treated with joint mobilization only. The treatment efficiency,the change of shoulder status before and after treatment and VAS of degree of pain index were compared between the two groups. Results The total effective rate of observation group was obviously higher than that of control group,the difference was statistically significant（ P 〈 0. 05）. The shoulder function ADL scores and ROM score and total score of ob-servation group after treatment were significantly higher than that of the control group,the difference was statistically significant（ P 〈 0. 05）. Af-ter treatment,the VAS score of observation group was obviously lower than that of control group,the difference was statistically significant（ P 〈0. 05）. Conclusion The utility of extracorporeal shock wave joint manipulation therapy can significantly improve the treatment efficiency,im-prove the patientsˊ shoulder function,reduce the patientsˊpain,which is better than pure joint manipulation therapy. Therefore,it is suggested for clinical use.
