外周血APPmRNA、血小板APP异构体比值辅助诊断阿尔茨海默病的临床价值

摘　　要：目的:基于外周血淀粉样前体蛋白(APP)mRNA、血小板APP130KD/APP106KD(APP异构体比值)检测,验证APPmRNA、血小板APP异构体比值作为辅助阿尔茨海默病(AD)早期临床诊断的价值.方法:依据相关诊断标准纳入轻度老年痴呆症病人28例,另选择年龄和文化程度相当、无神经系统疾病、简易智能状态检查(MMSE)≥27分、头颅CT或MRI检查证实无脑萎缩和脑梗死者30名作为健康对照组.分别检测患病组及健康对照组MMSE(简易智能状态检查)、外周血APPmRNA和血小板APP异构体比率.结果:外周血APPmRNA与老年痴呆症病人缺乏相关性;而血小板APP异构体比值与老年痴呆症则具有明显相关性,差异具有统计学意义(P<0.05).结论:血小板APP异构体比值与老年痴呆症具有相关性,其作为辅助AD早期临床诊断及客观评价AD病情的指标有一定应用价值.Aim：Based on measuring on APPmRNA and of platelet APP isomer ratio in peripheral blood, verify the early clinical value of the two as an assistant diagnosis of Alzheimer＇s disease. Methods： According to the relevant diagnostic criteria, 28 patients with mild dementia were included. Choose another 30 as a healthy control group which had equivalent age and education level and had not nervous system diseases,whose mini-mental state examination is more than or equal to twenty seven points and whose Brain atrophy and cerebral infarction were confirmed by computed tomography or magnetic resonance imaging examination. The mini-mental state examinations and APPmRNA and of platelet APP isomer ratios of the disease group and the healthy control group were detested respectively. Results： APPmRNA in peripheral blood lacks of association with Alzheimer＇s disease,yet platelet APP isomer ratio in peripheral blood has a significant correlation with Alzheimer＇s disease,and the difference was statistically significant（P〈0.05）. Conclusion：platelet APP isomer ratio in peripheral blood has a significant correlation with Alzheimer＇s disease.It has certain application value as an early clinical diagnosis assistant to Alzheimer＇s disease and objective evaluation index of Alzheimer＇s disease condition.
