彩色多普勒超声评价肾动脉狭窄介入治疗前后血流动力学变化的应用价值

摘　　要：目的探讨彩色多普勒超声评价肾动脉狭窄(renal artery stenosis,RAS)介入治疗前后血流动力学变化的价值。方法选择经彩超检查及肾动脉造影确诊肾动脉狭窄并行介入治疗患者36例,对治疗前后的肾动脉血流动力学参数、血压、血肌酐进行对比分析。结果介入治疗后肾动脉流速峰值(peak systolic velocity,PSV)、阻力指数(RI)、肾动脉与腹主动脉流速比值(RAR)、肾动脉与叶间动脉流速比值(RIR)明显降低,叶间动脉加速时间(AT)降低,与介入治疗前比较均有统计学意义(P<0.05);治疗前后叶间动脉PSV、RI比较无明显统计学意义(P>0.05),术后半年患者平均收缩压及舒张压较治疗前有明显差异(P<0.05)。结论应用彩色多普勒超声检测肾动脉血流动力学变化评价肾动脉狭窄介入治疗效果具有重要价值。Objective To evaluate the application value of color Doppler uhrasonography in evaluation of renal artery stenosis before and after interventional treatment. Methods 36 patients with renal artery stenosis, who were confirmed by renal artery angiography, were compared and analyzed before and after the interwentional treatment. Hemodynamic parameters of renal arteries, blood press and didney function were measured. Results The PSV, RI, RSR, RIR of the renal arteries, the AT of interlobar ar- tery and the press were markedly descreased （ P 〈 0.05 ）. Conclusion Color Doppler fuhrasonography has important application value in diagnosing renal artery stenosis and evaluating the effect of interwentional treatment.
