胎儿超声心动图在胎儿心律失常诊断中的应用体会

摘　　要：目的探讨胎儿超声心动图在胎儿心律失常诊断中的应用价值。方法选取200例疑似心律失常的胎儿（胎龄在14~42周）进行检查,均接受超声心动图检查,并将超声心动图检查结果与真实情况进行对比。结果 1超声检查结果检出阳性12例,阳性率为6.00%,阴性188例,阴性率为94.00%,真实情况有11例为阳性,阳性率为5.50%,189例阴性,阴性率为94.50%,对比差异无统计学意义（P〉0.05）。2胎儿超声心动图检测的特异度为75.00%、敏感度为98.94%、准确度为97.50%。3在11例心律失常患儿中,缓慢型心律失常1例、心房扑动3例、室性期前收缩3例及房性期前收缩4例。结论超声心动图在胎儿心律失常的诊断中有着较高的应用价值,便于尽早检出胎儿心律失常,及时采取处理措施。[Objective]To investigate the value of fetal echocardiography in the diagnosis of fetal arrhythmia.[Methods]Two hundred cases of suspected fetal arrhythmia（gestational age at 42 to 14 weeks） were examined,all patients underwent echocardiography,and the detection results were compared with the real situations.[Results]① The ultrasonic examination results were positive in 12 cases,positive rate was 6.00%,the results were negative in 188 cases,the negative rate was 94.00%,11 cases were positive in real situation,the positive rate was 5.50%,189 cases were negative,the negative rate was 94.50%,and the differences was not statistically significant（P〉0.05）.② The specificity of fetal echocardiography was 75%,sensitivity was 98.94%and accuracy was 97.50%.③ In 11 cases of fetal arrhythmias,there were 1 case of slow arrhythmia,3 cases of atrial flutter,3 cases of ventricular contraction and 4 cases of atrial extrasystole.[Conclusion]Echocardiography has a high application value in the diagnosis of fetal cardiac arrhythmia,which is convenient for the detection of fetal arrhythmia,and to take measures to deal with it in time.
