经筋微创疗法对兔骨性关节炎模型细胞因子及滑膜的病理研究

摘　　要：目的观察经筋微创疗法对木瓜蛋白酶诱导的兔膝骨性关节炎(osteoarthritis,OA)外周血清、关节液中炎症因子表达及关节内滑膜组织病理变化,探讨该疗法的抗炎机理。方法将20只雌性新西兰大白兔随机均分4组(治疗组、对照组、造模组、空白组),各5只。造OA模型(空白组不造模),治疗4周。实验结束后选兔耳缘静脉取血并抽取膝关节液,检测血清及关节液中白介素-1β(interleukin-1β,IL-1β)、肿瘤坏死因子-α(tumor necrosis factor,TNF-α)、转化生长因子-β1(transforming growth factor-β,TGF-β1)、小鼠基质金属蛋白酶-13(matrix metalloproteinase 13,MMP-13)含量,取关节内滑膜组织进行病理切片分析。结果兔外周血清及关节液中IL-1β、TNF-α、TGF-β1、MMP-13的含量,造模组和空白组相比差异有统计学意义(P<0.05);治疗4周后,对照组和治疗组较造模组相比差异有统计学意义(P<0.05);对照组与空白组相比差异具有统计学意义(P<0.05)。结论经筋微创疗法可有效降低兔膝骨性关节炎外周血及关节液中IL-1β、TNF-α、TGF-β1、MMP-13含量,减轻滑膜炎症。Objective To investgate the effect and mechanism of meridian-sinew release therapy for papain-induced rabbit knee osteoarthritis（OA）on peripheral blood serum,inflammatory factors expression of synovial fluid,and pathological variation of synovial tissue.Methods Twenty New Zealand white rabbits were randomly divided into four groups（n=5 in each group）：treatment group（A）,control group（B）,model group（C）,and blank group（D）.The rabbits in treatment group,control group and model group were modeled as knee osteoarthritis with the following4 weeks’ treatment.The blood and synovial fluid was separately obtained from rabbit ear side vein and knee after the final administration.The contents of interleukin-1β（IL-1β）,tumor necrosis factor（TNF-α）,transforming growth factor-β（TGF-β1）and matrix metalloproteinase 13（MMP-13）in the rabbits’ serum and synovial fluid were measured,and the synovial tissues of knee joint cavity were sliced for pathological analysis.Results Compared with model group and control group,levels of IL-1β,TNF-α,TGF-β1,MMP-13 were significantly differences in rabbit peripheral blood and synovial fluid（P〈0.05）.Control group and treatment group had a significant difference with model group（P〈0.05）,and there was significant difference between control group and blank group（P〈0.05）.Conclusion Meridian-sinew release therapy not only reduces effectively the content of IL-1β,TNF-α,TGF-β1 and MMP-13 in rabbits’ peripheral blood and synovial fluid but it also lightens the synovial inflammation.
