摘 要:目的了解变形菌属临床分离株中整合子的流行状况及其介导的耐药性,为加强临床对该菌属耐药性的监测提供依据。方法收集2012年1月-2013年12月临床分离的无重复变形菌属114株,采用聚合酶链反应(PCR)、PCR mapping和序列分析方法检测Ⅰ、Ⅱ、Ⅲ类整合子整合酶基因(intI1、intI2、intI3)和整合子可变区基因,应用琼脂二倍稀释法测定菌株对抗菌药物的敏感性。结果 114株变形菌属中奇异变形菌109株、普通变形菌5株;109株奇异变形菌中intI1、intI2、intI3检出率分别为56.9%、48.6%、0,5株普通变形菌中intI1、intI2、intI3均未检出;dfr和aad基因阳性株对甲氧苄啶和氨基糖苷类的耐药率分别高于dfr和aad基因阴性株(P<0.05)。结论Ⅰ、Ⅱ类整合子在临床分离奇异变形菌中广泛流行,其基因盒上携带的耐药基因与相应抗菌药物的耐药性密切相关,临床上宜重视其耐药性监测;首次在奇异变形菌Ⅰ类整合子中发现基因盒aadB-orf1-cmlA5和基因盒aadA23。OBJECTIVE To investigate the prevalence of integrons and the antimicrobial resistance of clinical isolates of integrons-mediated in Proteus spp,so as to provide the evidence of intensively monitoring their antimicrobial resistance in clinic.METHODS A total of 114 isolates of nonrepetitive Proteus spp.were collected from Jan.2012 to Dec.2013.Integrase gene intI1,intI2,and intI3 of classⅠ,Ⅱ,Ⅲinegrons,and variable region gene of integrons were detected by PCR,PCR mapping and sequencing.Antimicrobial susceptibility were tested by agar double dilution method.RESULTS There were 109 strains of Proteus mirabilis and 5strains of Proteus vulgaris in this study.The detection rates of strains harboring intI1,intI2,and intI3 gene were 56.9%,48.6%,0.0%in P.mirabilis isolates,respectively,while all these integrase genes were not found in 5strains of P.vulgaris.The antimicrobial resistant rates of Proteus mirabilis with dfr and aad gene to trimethoprim and aminoglycosides were significantly higher than those of isolates without dfr and aad gene respectively(P〈0.05).CONCLUSIONClassⅠandⅡintegrons are prevalent in P.mirabilis clinical isolates,and the antimicrobial resistance is associated closely with the resistant gene of gene cassettes of integrons,and then the antimicrobial resistance surveillance of Proteus mirabilis should be stressed.This is the first report that gene casstte aadB-orf1-cmlA5 and gene casstte aadA23 exist in classⅠintegrons of P.mirabilis.
