摘 要:目的探讨晚期肺癌患者肺部感染病原菌分布和药敏试验结果,为临床治疗提供参考依据。方法收集2014年3月-2015年5月医院收治的174例晚期肺癌并发肺部感染患者临床资料,收集全部患者痰液标本进行细菌培养,分析感染病原菌类型分布和药敏结果,数据采用SPSS 11.0软件进行统计分析。结果共培养出237株病原菌,以革兰阴性菌为主,共144株,占60.76%,其次真菌,共51株,占21.52%,革兰阳性菌共42株,占17.72%;肺炎克雷伯菌、鲍氏不动杆菌、铜绿假单胞菌对亚胺培南耐药率低,均<10.00%;金黄色葡萄球菌对左氧氟沙星、庆大霉素和磺胺甲噁唑/甲氧苄啶耐药率为25.00%。结论肺癌晚期患者肺部感染病原菌种类繁多,临床应根据药敏结果合理应用抗菌药物。OBJECTIVE To investigate the pathogen distribution and bacterial sensitivity test results in patients with advanced lung cancer,so as to provide the basis for clinical treatment.METHODS A total of 174 cases of pulmonary infection in patients with advanced lung cancer clinical data admitted to hospital from Mar.2014 to May 2015 were collected.All patients were collected for bacterial cultures of sputum specimens to analyze in patients the pathogen type distribution and drug susceptibility results.SPSS 11.0was used for data analysis.RESULTS A total of 237 strains of pathogens were cultured,including mainly 144 strains of gram-negative bacteria,accounting for 60.76%,51 strains of fungi,accounting for 21.52%,and 42 strains of gram-positive bacteria,accounting for17.72%.Susceptibility results showed Klebsiella pneumoniae,Acinetobacter baumannii,Pseudomonas aeruginosa to imipenem lower than 10.00%.Staphylococcus aureus to levofloxacin,gentamicin and sulfamethoxazole/trimethoprim had resistance rate of 25.00%.CONCLUSIONThere are wide variety of pathogens caused lung infection in patiens with advanced lung cancer.The clinical use of antibiotics should be reasonable based on susceptibility results.
