综合性医院骨科医院感染危险因素分析

摘　　要：目的分析综合性医院骨科医院感染状况及危险因素,为预防和控制医院感染提供理论依据。方法利用医院感染实时监控系统提取2013年1月-2014年12月骨科住院患者17 561例患者进行回顾性分析,其中感染221例,分析其感染因素,数据采用SPSS 13.0软件进行统计分析。结果 17 561例患者中,有221例患者发生感染,感染率为1.26%;发生感染258例次中,感染部位以下呼吸道为主,共71例次占27.52%;logistic回归分析显示,年龄、住院天数及使用中心静脉插管是骨科医院发生感染的独立危险因素(P<0.05),性别为保护性因素。结论骨科医院感染危险因素较多,应积极治疗基础疾病、缩短住院时间、手术时间和术前住院时间、减少侵入性操作及改善环境等,加强预防和控制,以降低医院感染率,提高医疗质量。OBJECTIVE To determine the incidence and risk factors of nosocomial infections（NI）in orthopedics department of the general hospital,in order to provide the theory basis for prevention and control of NI.METHODS By using the real-time nosocomial infection surveillance system,17561 patients in the department of orthopedics from Jan.2013 to Dec.2014 were selected and retrospectively reviewed,including 221 cases of infections.The infection factors were analyzed,and the data were analyzed by SPSS 13.0.RESULTS In the survey of 17561 patients,there were 221 patients with NI and the infection rate was 1.26%.The frequency of infection was 258 and the infection site was mainly lower respiratory tract.Logistic regression analysis showed that age,hospitalization days,and the use of central venous catheter were the independent risk factors of NI（P〈0.05）,and gender was the protective factor.CONCLUSIONThe risk factors of NI in the orthopedics department are varied.We should try to treat basic diseases,shorten the hospitalization days,the operation time and preoperative hospitalization days,reduce invasive operation and improve the environment for the sake of reducing the incidence of nosocomial infections.We also should strengthen the prevention and control of hospital infections in order to improve the quality of medical care.
