摘　　要：目的探讨人感染H7N9禽流感病毒病临床表现及诊断和治疗,改善患者预后。方法分析1例人感染H7N9禽流感病毒患者的临床表现、影像学特点和诊治过程,并检索2013-2014年两年的文献进行复习。结果该例为男性患者,43岁,以发热为首发症状,发病迅速,最后进展为重症肺炎,予抗病毒、抗感染、机械通气等治疗后好转;文献检索到H7N9患者绝大部分为重症病例,该病从发病到入院、实验室确诊、出院、死亡的中位时间分别是4.5、8.3、41.7、11d,推荐NA抑制剂药物进行抗病毒治疗。结论绝大多数H7N9患者病情危重,早期确诊及尽早使用奥司他韦等积极治疗对挽救患者生命、改善预后具有重要价值。OBJECTIVE To explore the clinical manifestation,diagnosis and treatment of infection of H7N9 avian influenza virus disease,so as to improve patients prognosis.METHODS The clinical manifestation,radiological findings,diagnosis and treatment process in one case of H7N9 from our hospital were analyzed,and the literatures in 2013-2014 were searched and reviewed.RESULTS The case was a middle-aged male patient,with fever as the first symptom,rapid progression to severe pneumonia at last.Anti-virus,anti infection and mechanical ventilation were used and condition got improved.Most of H7N9 patients from literature review were severe cases.The median time of the disease from the onset to hospital,laboratory diagnosis,discharge,death were 4.5days,8.3days,41.7days and 11 days,NA inhibitor drugs were recommended for antiviral therapy.CONCLUSION Most of H7N9 patients are in critical condition.Early diagnosis and as soon as possible to use of oseltamivir and other active treatments have significant values to save the patients lives and improve outcomes.
