摘 要:目的探讨血清可溶性髓细胞触发受体-1(sTREM-1)和降钙素原(PCT)对围生期宫内感染合并肺炎新生儿肺出血(NPH)的影响,为围生期宫内感染合并肺炎NPH的预防治疗提供临床依据。方法随机选择2013年12月-2015年9月医院收治围生期宫内感染合并肺炎的肺出血新生儿60例为观察组,以及非肺炎肺出血新生儿60例为对照组,对两组患儿实施sTREM-1和PCT检测,针对两组患儿的sTREM-1、PCT含量,sTREM-1和PCT与母体围生期疾病的关系等指标进行观察比较。结果观察组sTREM-1和PCT水平均高于对照组患儿,差异有统计学意义(P<0.05);NPH肺炎患儿sTREM-1和PCT水平均高于NPH呼吸窘迫综合征、缺氧缺血性脑病、颅内出血患儿,差异有统计学意义(P<0.05);围生期母体原发病宫内感染NPH患儿sTREM-1和PCT水平高于围生期母体疾病妊高症、羊水早破、胎盘早剥、糖尿病、其他疾病NPH患儿,差异有统计学意义(P<0.05)。结论在临床上实施sTREM-1和PCT监测,可以对围生期母体原发病宫内感染与患儿肺炎NPH的发生和发展进行有效预测,具有较高的临床应用价值。OBJECTIVE To research the impact of serum soluble myeloid cells trigger receptor 1(sTREM-1)and procalcitonin(PCT)on pulmonary hemorrhage in newborn with intrauterine infection in the perinatal period combined pneumonia,so as to provide clinical basis for pulmonary hemorrhage prevention in newborn with intrauterine infection in the perinatal period combined pneumonia.METHODS A total of 60 cases of children with neonatal pulmonary hemorrhage with intrauterine infection in the perinatal period combined pneumonia in our hospital during Dec.2013 to Sep.2015 were randomly selected as observation group,another 60 patients with pneumonia but without pulmonary hemorrhage as control group.Two groups of children were provided sTREM-1and PCT test,and the levels of sTREM-1and PCT,and its relation to the maternal diseases in the perinatal period were observed.RESULTS In this study,the levels of sTREM-1and PCT in observation group of patients were significantly higher than those in the control group(P〈0.05).The sTREM-1and PCT levels in children with NPH were significantly higher than those in children with NPH respiratory distress syndrome,ischemia anoxic encephalopathy and intracranial hemorrhage(P〈0.05).The sTREM-1and PCT levels in children with pulmonary hemorrhage and intrauterine infection in the perinatal period were significantly higher than those in NPH children with pregnancy hypertension in mother,premature rupture of the amniotic fluid,placental abruption,diabetes and other diseases(P〈0.05).CONCLUSIONThe monitoring of sTREM-1and PCT in clinic could give a predict for occurrence and development of intrauterine infection in the perinatal period combined pneumonia in children,which has a high clinical value.
