摘 要:目的对孕妇携带的大环内酯类耐药的B群链球菌(GBS)进行多位点序列分型及大环内酯类耐药基因检测,探讨其多位点序列型与大环内酯类耐药基因的相关性。方法采用PCR方法检测东莞地区孕妇携带的82株大环内酯类耐药GBS的耐药基因erm B、mef A/E、lin B。采用PCR扩增82株大环内酯类耐药GBS的7个管家基因,对其进行DNA测序,并对测序结果进行多位点序列分型(MLST),以e BURSTv3.0分析菌株间的亲缘关系。结果 82株大环内酯类耐药的GBS中,耐药基因erm B、mef A/E和lin B的总检出率分别为73.17%、46.34%和7.32%。MLST分型得到CC1、CC12、CC17、CC19共4个克隆群;17个序列型,主要序列型为ST1(6.10%)、ST12(7.32%)、ST17(9.76%)、ST19(37.8%)和ST23(14.63%)。结论东莞地区孕妇携带的序列型ST19的GBS检测率高;序列型ST12、ST19、ST23的GBS大环内酯类耐药基因携带率高,其分子构型可能与大环内酯类耐药基因的获得关系密切。Objective To explore the relevance of the multilocus sequence type and macrolide-resistant gene by detecting the multilocus sequence type and maerolide-resistant gene of macrolide-resistant Group B Streptococcus (GBS) in pregnant women. Methods The macrolide-resistance genes such as ermB, mefA/E, linB and seven housekeeping genes used for GBS characterization in 82 GBS strains were amplified by PCR. The PCR products were analyzed by DNA sequencing. Alleles for 7 housekeeping genes were assigned on the MLST website and each isolate was defined by the sequence typing. The eBURSTv3.0 software was used to analyze the genetic relationships of different strains. Results In 82 GBS strains, 73.17% isolates carried the ermB gene, 46.34% carried mefA/E gene and 7.32% carried linB gene. After MLST analyzed, received 4 CCs, including CC1, CC12, CC17, CC19; and 17 STs were acquired, the detection rates in all strains of the main sequence types containing ST1, ST12, ST17, ST19 and ST23 were 6.10%, 7.32%, 9.76%, 37.8% and 14.63%, respectively. Conclusion The most frequent sequence type of GBS from pregnant woman in Dongguan is ST19; Sequence type ST12, ST19 and ST23 have high detection rates of macrolide-resistant gene, their molecular structure may closely relate to the acquisition of macrolide-resistant gene.
