晚期食道癌患者治疗前后CD^(3+)、CD^(4+)、CD^(8+)、CD25调节T细胞亚群检测分析

摘　　要：目的通过检测食道癌患者治疗前后外周血T细胞亚群表达水平,了解患者机体免疫功能状态。方法选用河北医科大学第四医院食道癌患者123例,其中淋巴结转移77例,未转移46例。抽取患者空腹外周血3ml,EDTA抗凝。采用流式细胞仪测定外周血CD3+,CD4+,CD8+,CD4+CD25+细胞亚群水平。结果治疗前CD3+,CD4+,CD8+,CD4+CD25+淋巴结转移和未转移组分别为58.22±6.70、29.79±3.31、21.81±3.31、3.34±1.31和56.43±8.35、28.75±4.62、23.99±3.93、4.44±1.48;治疗后分别为66.99±7.46、38.42±3.86、20.69±2.50、5.07±0.79和64.80±9.89、38.03±5.40、21.19±3.49、4.82±1.32。治疗前与治疗后组间比较差异无统计学意义(P>0.05)。CD4+治疗前后差异有统计学意义(P<0.05)。治疗前后CD4/CD8比值分别为1.26和1.82。讨论CD3+、CD4+、CD4+CD25+T治疗后略有上升,以CD4+上升较明显,而CD8+T略有下降,表明食道癌患者通过治疗,机体细胞免疫功能得到一定改善。Objectives To examine levels of peripheral blood T cell subsets in patients with esophageal cancer prior to treatment and to ascertain the immune status of patients in order to improve their prognosis and quality of life. Method Samples were collected from 123 patients with esophageal carcinoma who were seen at the Fourth Hospital of Hebei Medical University.Seventy-seven of the 123 patients had lymph node metastasis while 46 patients had no metastasis.Three ml of peripheral blood was collected from each patient with esophageal cancer and the blood was anticoagulated with EDTA.Levels of peripheral blood CD^3＋,CD^4＋,CD^8＋,and CD^25＋T cell subsets were measured using flow cytometry. Results Prior to treatment,levels of peripheral blood CD^3＋,CD^4＋,CD^8＋,and CD^25＋T cell subsets in patients with lymph node metastasis were 58.22±6.70,29.79±3.31,21.81±3.31,and 3.34±1.31,respectively,while levels of those T cell subsets in patients with no metastasis were 56.43±8.35,28.75±4.62,23.99±3.93,and 4.44±1.48.After treatment,levels of peripheral blood CD^3＋,CD^4＋,CD^8＋,and CD^25＋T cell subsets in patients with lymph node metastasis were 66.99±7.46,38.42±3.86,20.69±2.50,and 5.07±0.79,respectively,while levels of those T cell subsets in patients with no metastasis were 64.80±9.89,38.03±5.40,21.19±3.49,and 4.82±1.32.Levels of peripheral blood T cell subsets did not differ significantly before and after treatment.Levels of the CD^4＋T cell subset did differ significantly before and after treatment.The CD4/CD8 ratio was 1.26 before treatment and 1.82 after treatment. Conclusion Levels of CD^3＋and CD^25＋T cells increased slightly after treatment with levels of CD^4＋T cells increasing markedly.In contrast,levels of CD^8＋T cells decreased slightly.These findings indicate that the cellular immunity of patients with esophageal cancer improved somewhat after treatment.
