单孔直角腹腔镜腹膜后入路术式对感染坏死性胰腺炎患者的疗效及安全性影响

摘　　要：目的研究单孔直角腹腔镜腹膜后入路术式对感染坏死性胰腺炎患者的疗效及安全性影响。方法回顾性分析我院2010年1月~2015年12月收治的感染性坏死胰腺炎患者64例作为研究对象,将单孔直角腹腔镜腹膜后入路术式的有患者作为观察组,将实施传统开腹治疗的患者作为对照组,观察两组患者的疗效及安全性。结果对比两组患者各项手术指标,结果观察组患者术中出血量、手术时间、住院时间显著少于对照组,治疗前与治疗后1d两组患者APACHE-Ⅱ评分无明显差异,治疗后3d和7d的观察组APACHE-Ⅱ评分显著低于对照组,差异有明显的统计学意义（P〈0.05）。术后观察组并发症例数显著少于对照组,以上差异均有明显的统计学价值（P〈0.05）。结论采用单孔直角腹腔镜腹膜后入路术式对感染坏死性胰腺炎患者具有十分显著的疗效,坏死组织的清除率和手术安全性较高,值得临床推广应用。Objective To study the efficacy and safety of single port laparoscopic retroperitoneal approach to patients with necrotizing pancreatitis. Methods 64 cases of patients with necrotizing pancreatitis cured in our hospital from January 2010 to December 2015 were retrospectively analyzed. Patients with single port laparoscopic retroperitoneal approach were selected as observation group, and patients with traditional open surgery were selected as control group.The efficacy and safety of the two groups were observed. Results Comparing the 0peration indexes of the two groups, the amount of bleeding, operation time, length of stay of observation group were significantly less than those of control group.There was no significant difference on APACHE- Ⅱ score between the two groups before and 1d after the treatment.APACHE- Ⅱ scores of observation group 3d and 7d after the treatment were significantly lower than those of control group, and the differences were statistically significant（P〈0.05）.Number of complications of observation group after the operation was significantly less than that of control group, and all the differences above were statistically significant（P〈0.05）. Conclusion The efficacy of single port laparoscopic retroperitoneal approach to patients with necrotizing pancreatitis is very significant. Its removal rate of necrotic tissue and operation safety are high. It is worthy of clinical popularization and application.
