聚乙二醇干扰素a-2a联合阿德福韦酯治疗高病毒载量HBeAg阳性慢性乙型肝炎60例临床研究

摘　　要：目的:探讨聚乙二醇干扰素a-2a联合阿德福韦酯治疗高病毒载量HBeAg阳性慢性乙型肝炎的效果。方法:选取高病毒载量HBeAg阳性慢性乙型肝炎患者60例,按随机数表法分为观察组30例和对照组30例,对照组采用口服阿德福韦酯,10mg/次,1次/d,观察组采用口服阿德福韦酯联合聚乙二醇干扰素a-2a皮下注射,阿德福韦酯10mg/次,1次/d,聚乙二醇干扰素a-2a皮下注射180"g/次,1次/周,两组均治疗48周,治疗后对比分析两组AST、TBIL、ALB,HBV-DNA、HBsAg阴转率指标。结果:治疗后两组患者的AST、TBIL值均下降,观察组治疗后AST、TBIL值分别降至41.55±1.35U/L、21.11!3.65"moL/L,ALB值变为35.48!4.37g/L。对照组治疗后AST、TBIL、ALB对应值为52.35±2.55U/L、33.41!3.65"moL/L、34.22!3.51g/L,观察组患者ALB浓度较对照组升高,差异具有统计学意义(P<0.05)。治疗后观察组患者HBV-DNA、HBsAg阴转率分别为90%、70%,对照组分别为47%、37%,观察组患者HBV-DNA、HBsAg阴转率明显高于对照组,差异具有统计学意义(P<0.05)。结论:聚乙二醇干扰素a-2a联合阿德福韦酯对治疗高病毒载量HBeAg阳性慢性乙型肝炎有很好的效果,值得临床推荐。Objective：To inquiry pegylated interferon a-2acombined with adefovir dipivoxil therapy with high viral load HBeAg positive chronic effects.Methods：60patients with high viral load HBeAg positive chronic hepatitis B in our hospital were selected to be randomly divided into observation group of 30 cases and a control group of 30 cases according to the random number table method.Observation group＇patients took oral adefovir peginterferon a-2asubcutaneous injection.Adefovir 10mg/times,1times/d,peginterferon a-2asubcutaneously 180g/second,1/w;The control group＇patients took only Adefovir,10mg/times,1times/d,two groups＇ treatment was totally 48 weeks.Results：AST,TBIL value of the two groups＇ patients was declined after treatment,AST,TBIL value of the observation groups＇ patients decreased to 41.55±1.35U/L,21.11 ±3.65 μmoL/L after treatment,ALB value became 35.48±4.37g/L.The AST,TBIL,ALB value of the control group＇patients became 52.35±2.55U/L,33.41±3.65μmoL/L,34.22!3.51g/L,and the ALB＇concentration of observation group was higher than the control group,the difference was statistically significant（P〈0.05）.The HBV-DNA,HBsAg negative rate of observation group＇patients were 90% and 70% after treatment,and the HBV-DNA,HBsAg negative conversion rate of control group＇patients were 47%,37%.Observation group＇HBV-DNA,HBsAg negative conversion rates were significantly higher,the difference was statistically significant（P〈0.05）.Conclusion：Adefovir joint peginterferon a-2athan single adefovir dipivoxil for the treatment of viral load HBeAg positive chronic hepatitis B have a better effect.
