耳穴贴压法用于潜伏期分娩镇痛的临床研究

摘　　要：目的观察耳穴贴压法用于潜伏期分娩镇痛的临床应用效果。方法将200例足月单胎初产妇,按单双日随机分为观察组(单日)和对照组(双日)各100例。观察组在潜伏期行耳穴贴压法分娩镇痛,对照组潜伏期无任何镇痛方法,两组在活跃期均行硬膜外分娩镇痛。比较两组产妇的中转剖宫产率及新生儿Apgar评分,记录第一产程时间、潜伏期及活跃期疼痛评分、产妇镇痛满意度。结果观察组中转剖宫产率为4%,对照组中转剖宫产率为5%,两组比较差异无统计学意义(P>0.05);观察组新生儿1 min与5 min的Apgar评分分别为(8.63±0.88)分、(9.15±0.71)分,对照组新生儿1min与5min的Apgar评分分别为(8.72±0.98)分、(9.13±0.80)分,两组比较差异无统计学意义(P>0.05);第一产程时间观察组为(361±32.4)min,对照组为(432±39.4)min,两组比较差异有统计学意义(P<0.05);对全产程的镇痛满意度,观察组满意率为91%、对照组为71%,两组比较差异有统计学意义(P<0.05);观察组潜伏期VAS评分为(2.21±0.75)分,对照组(4.91±1.12)分,两组比较差异有统计学意义(P<0.05),观察组活跃期VAS评分为(2.14±0.56)分,对照组为(2.15±0.51)分,两组比较差异无统计学意义(P>0.05)。结论耳穴压贴法用于潜伏期分娩镇痛时镇痛作用明显,可缩短第一产程,不增加剖宫产率,对新生儿Apgar评分无影响。Objective To observe the clinical effect of auricular acupoint plaster therapy applied in labor analgesia in latent phase.Method Divide 200full-term monomorphic primiparaes into observation group（odd date）and control group（even date）randomly.Primiparaes in observation group receive auricular acupoint plaster therapy in latent phase.Primiparaes in control group receive no analgesia method in latent phase.Patients in two groups all receive epidural labor analgesia in active phase.Compare the cesarean section rate and Apgar score of the newborn of two groups.Record the time of the first stage of labor,pain score in latent and active phase and satisfaction level of analgesia.Result Cesarean section rate is4%in observation group and 5%in control group.There is no significant difference between two groups（P〉0.05）.Apgar scores of 1min and 5min for newborn in observation group are（8.63±0.88）and（9.15±0.71）.Apgar scores of 1min and5 min for newborn in observation group are（8.72±0.98）and（9.13±0.80）.There is no significant difference between two groups（P〉0.05）.Time of the first stage of labor is（361±32.4）in observation group and（432±39.4）in control group.There is significant difference between two groups（P〈0.05）.Satisfaction level of whole stages of labor is 91%in observation group and 71%in control group.There is significant difference between two groups（P〈0.05）.VAS score is（2.21±0.75）for observation group and（4.91±1.12）for control group in latent phase.The difference is significant（P〈0.05）.VAS score is（2.14±0.56）for observation group and（2.15±0.51）for control group in active phase.There is no significant difference between two groups（P〉0.05）.Conclusion Auricular acupoint plaster therapy has obvious effect on labor analgesia in latent phase,which can shorten time of the first stage of labor and won＇t increase cesarean section rate or influence newborns＇ Apgar score.
