摘 要:目的了解医院2013年住院患者医院感染率的情况,为医院感染管理提供参考依据。方法采用医院感染实时监控系统自动搜索疑似感染病例,同时结合临床医师主动上报医院感染病例,对疑似感染病例由临床经管医师进行核对确认,医院感染管理科最终确认并进行统计分析,得出医院感染信息及结果。结果 45 156例住院患者发生医院感染1 047例,感染率2.3%;医院感染1 209例次,医院感染例次率为2.7%。科室感染情况,依次为重症监护病房(ICU)(15.9%)、血液科(11.9%)、急诊科(10.8%)、神经外科(7.8%)、心胸外科(5.3%)、神经内科(4.2%)、小儿外科(3.1%)、骨科(3.0%)、风湿免疫科(3.0%);感染部位以下呼吸道感染(33.2%)占首位,其次是上呼吸道、泌尿道、器官腔隙等部位感染。常见的前5位感染病原菌依次为肺炎克雷伯菌(20.3%)、大肠埃希菌(16.4%)、鲍曼不动杆菌(12.7%)、凝固酶阴性葡萄球菌(10.2%)和铜绿假单胞菌(10.0%)。结论医院感染的高发科室主要为ICU、血液科和急诊科,主要感染部位为下呼吸道,常见的病原体为肺炎克雷伯菌、大肠埃希菌和鲍曼不动杆菌。Abstract: Objective To survey the prevalence of hospital infections in hospitalized patients during 2013 for better control of hospital infections. Methods During the period from January 1, 2013 to December 31, 2013, the suspected cases of hospital infection were searched and reported via a real-time monitoring system in a tertiary general hospital. These. cases and the non-solicited cases reported by clinical doctors were subsequently reviewed and confirmed by the doctor-in-charge and finally adjudicated by the Department of Hospital Infection Management. The data were subjected to descriptive statistical analysis. Results A total of 1 209 cases of hospital infections were identified from 45 156 inpatients during 2013. The one-year prevalence of hospital infections was 2.7% (1 209/ 45 156). The hospital infections were reported in 1 047 (2.3%) of all the patients, mainly in ICU (15.9%), followed by Department of Hematology (11.9%), Emergency Room (10.8%), Department of Neurosurgery (7.8%), Cardiothoracic Surgery (5.3%), Neurology (4.2%), Pediatric surgery(3.1%), Orthopedics (3.0%), and Department of Rheumatology (3.0%). The primary site of infection was lower respiratory tract (33.2%), upper respiratory tract and urinary tract. The top five pathogens were Klebsiella pneumoniae (20.3%), E. coli (16.4%) and Aeinetobaeter baumannii (12.7%), coagulase-negative Staphylococcus (10.2%) and Pseudomonas aeruginosa (10.0%).Conclusions The hospital infections identified in this hospital during 2013 are primarily from ICU, Department of Hematology and Emergency Room. The most common site of infection is lower respiratory tract and common pathogens are Klebsiella pneumOniae, E. coli and Acinetobacter baumannii.
