摘 要:目的探讨老年冠状动脉粥样硬化性心脏病(coronary atherosclerotic heart disease,CAD)患者血清中同型半胱氨酸(homocysteine,Hcy)浓度与血脂浓度的相关性。方法选取广西壮族自治区人民医院老年冠心病患者60例纳入CAD组,于健康饮食后3d清晨空腹采血,检测血清中Hcy、三酰甘油(triacylglycerol,TG),总胆固醇(total cholesterol,TC),低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C),高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、脂蛋白a[lipoprotein a,LP(a)]浓度。同时选取我院健康体检者60名纳入对照组,检测相应指标,与实验组进行对比分析。结果 CAD组血清Hcy、TG、TC、LDL-C、LP(a)浓度均明显高于对照组,差异有统计学意义(P<0.05);HDL-C浓度明显低于对照组,差异有统计学意义(P<0.05)。CAD组中Hcy与LP(a)呈明显的正相关趋势(r=0.779,P=0.001),与HDL-C存在明显的负相关(r=-0.704,P=0.001;对照组人群Hcy值与各项指标之间均不存在明显的相关性。结论患者血清Hcy、Lp(a)浓度越高,发生CAD的风险越大,会进一步使体内HDL-C浓度降低。因此,高Hcy,高Lp(a)和低HDL-C浓度对患CAD的风险警示有着重要意义。Objectives To investigate the correlation between homocysteine (Hcy) and blood lipid in elderly patients with coronary atherosclerotic heart disease (CAD). Methods Totally 60 elderly patients with CAD in People's Hospital of Guangxi Zhuang Autonomous Region were selected as CAD group. Fasting venous blood of the 60 patients was collected in the morning in 3 days' healthy diet, and serum concentrations of Hcy, triacylglycerol (TG), (total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C) and lipoprotein a [LP (a) ] were detected. At the same time, 60 healthy cases undergoing physical examination in our hospital were selected as control group. The same indicators were detected in control group, and then compared with those in experimental group. Results Serum concentrations of Hcy, TG, TC, LDL-C and LP (a) in CAD group were significantly higher than those in control group (P〈O.05). Serum concentration of HDL-C was significantly lower in CAD group than in control group (P〈0.05). There was a positive correlation between Hcy and Lp(a) (r=0.779, P= 0.001〈0.05), but a negative correlation between Hcy and HDL-C (r=-O.704,P=O.O01〈O.05) in CAD group. There were on significant correlations between Hcy and other indicators such as TG,TC,LDL-C,HDL-C and LP (a) in control group. Conclusions The higher serum concentrations of Hcy and Lp (a) are, the greater CAD risk exists, which further reduces serum concentration of HDL-C. Therefore, high concentrations of Hcy, Lp (a) and low concentration of HDL-C play an important role in CAD prediction.
