撬拨复位中空钉内固定与切开复位钛板内固定对SandersⅡ型跟骨骨折的早期疗效观察

摘　　要：目的比较撬拨复位中空钉内固定与切开复位钛板内固定对SandersⅡ型跟骨骨折的临床疗效及安全性。方法根据患者入院顺序将80例SandersⅡ型跟骨骨折患者随机分为两组,各40例。对照组行切开复位钛板内固定治疗,观察组行撬拨复位中空钉内固定治疗,观察两组临床效果。结果观察组患者术中出血量少于对照组（P〈0.01）,手术时间及住院时间均短于对照组（P〈0.01）;两组术后随访12-26（19.54±3.42）个月,骨折愈合时间、Maryland功能评分、Gissane及Bhler角比较,差异均无统计学意义（P〉0.05）;观察组并发症发生率低于对照组（P〈0.05）。结论撬拨复位中空钉内固定与切开复位钛板内固定治疗SandersⅡ型跟骨骨折均具有较好效果,但撬拨复位中空钉内固定出血少,术后恢复快、并发症少。Objective To compare the clinical efficacy and safety of hollow screw fixation under reduction by leverage and titanium plate fixation under open reduction and internal fixation for SandersⅡ. Methods Eighty cases of Sanders Ⅱ calcaneus fracture were randomly divided into two groups（ either 40 cases）. Control group were treated with titanium plate fixation under open reduction and internal fixation; while 40 cases in the observation group were treated with hollow screw fixation under reduction by leverage. Two groups of clinical indicators of differences were compared. Results All the two groups were followed up for 12 - 26 months,average（ 19. 54 ± 3. 42） months. And compared to the control group,observation group,the intraoperative bleeding volume,operation time and hospital stay were significantly shorter,the incidence rate of complication was decreased significantly（ P〈0. 05）,but two groups of fracture healing time,Maryland function score,gissane and Bhler angle were compared with no statistical significance（ P〈0. 05）.Conclusion Two methods in the treatment of Sanders Ⅱ fractures both have a good effect,but the hollow screw fixation under reduction by leverage had less bleeding,postoperative recovery and less complications.
