摘 要:目的探讨急性冠状动脉综合征(ACS)患者外周血单核细胞(PBMC)微小RNA-24(miR-24)表达与细胞凋亡及炎性因子的相关性。方法选择在首都医科大学附属北京中医院住院治疗的稳定性心绞痛(SAP)组33例,不稳定性心绞痛(UAP)组42例,急性心肌梗死(AMI)组32例,另选择20例胸痛综合征(CPS)患者(CPS组)。采集患者外周静脉血并分离PBMC,RT-PCR检测PBMC miR-24表达,ELISA检测血浆可溶性Fas(sFas)、可溶性Fas配体(sFasL)、白细胞介素6(IL-6)、TNF-α。并采用Gensini积分法对各冠状动脉狭窄程度及狭窄节段做定量评分。结果 AMI组PBMC miR-24表达显著低于SAP组(1.68±0.41 vs 3.78±0.38,P=0.035);随着冠状动脉病变程度加重,患者miR-24表达逐渐降低;血浆sFas(r=-0.630,P=0.006)、sFasL(r=-0.372,P=0.015)、IL-6(r=-0.642,P=0.006)、TNF-α(r=-0.596,P=0.008)与miR-24水平均呈显著负相关。结论 ACS患者外周血miR-24表达水平下调,且与细胞凋亡及炎性因子水平呈显著负相关。Objective To study the association of miR-24 expression in peripheral blood mononuclear cells(PBMC)with apoptosis and inflammatory cytokines in ACS patients.Methods One hundred and seven CHD patients admitted to our hospital were divided into stable angina pectoris(SAP)group(n=33),unstable angina pectoris group(n=42),acute myocardial infarction(AMI)group(n=32),and 20 chest pain syndrome(CPS)patients served as CPS group.PBMC were isolated from peripheral veinous blood samples.Expression of miR-24 in PBMC was detected by RTPCR.Serum levels of sFas,sFasL,IL-6and TNF-αwere measured.Coronary artery stenosis and stenosed segments were scored according to the Gensini Scoring System.Results The expression level of miR-24 in PBMC was significantly lower in AMI group than in SAP group(1.68±0.41 vs 3.78±0.38,P=0.035).The expression level of miR-24 deceased with the aggravation of coronary artery lesions.The serum levels of sFas,sFasL,IL-6and TNF-αwere negatively associated with the expression level of miR-24(r=-0.630,P=0.006;r=-0.372,P=0.015;r=-0.642,P=0.006;r=-0.596,P=0.008).Conclusion The level of miR-24 in PBMC is down-regulated in ACS patients and is negatively associated with apoptosis and inflammatory cytokines.
