胎儿血管环的产前超声诊断及临床价值

摘　　要：目的:探讨产前超声诊断胎儿血管环的临床价值。方法:回顾性分析8例产前超声诊断为血管环胎儿的超声图像与随访结果,总结其声像图特点及结局。结果:本组中2例双主动脉弓构成"O"形完全性血管环,5例右位主动脉弓、左位动脉导管、迷走左锁骨下动脉构成"U"形完全性血管环,1例左位主动脉弓、迷走右锁骨下动脉构成"C"形部分性血管环。结论:产前超声诊断胎儿血管环具有重要的临床价值。三血管气管切面是产前超声诊断胎儿血管环的最重要切面,辅以气管分叉冠状切面、主动脉弓长轴切面,能够对不同类型血管环进行较准确的诊断。Objective： To investigate the clinical value of fetal echocardiography in prenatal diagnosis of vascular ring. Methods： Ultrasound images and follow-up results of 8 fetuses with vascular ring were retrospectively analyzed and summa- rized. Results： Double aortic arch formed ＂O＂ shaped complete vascular ring in 2 cases. Right aortic arch, left ductus artery and aberrant left subclavian artery formed ＂U＂ shaped complete vascular ring in 5 cases. Left aortic arch and aberrant right subclavian artery formed ＂C＂ shaped incomplete vascular ring in one case. Conclusion： Fetal echocardiography is of great clinical value in prenatal diagnosis of vascular ring. Three vessels and trachea view is the most important for prenatal ultra- sonic diagnosis of fetal vascular ring. Tracheal bifurcation coronal view and long axial of aortic arch view is auxiliary view. The above views may make accurate diagnosis for the different types of fetal vascular ring.
