摘  要:目的探讨在大鼠膝骨性关节炎模型中辛伐他汀对其关节炎指数(AI)、胸腺指数、膝关节伸直方向活动度及血清细胞因子的影响。方法 60只大鼠随机分为正常对照组、模型组、阳性对照组、辛伐他汀组,每组15只。正常对照组灌胃等体积生理盐水,其余各组通过大鼠膝关节腔内注入木瓜蛋白酶制作骨关节病变模型。大鼠造模成功后,模型组给予等体积生理盐水;阳性对照组给予扶他林混悬液10 ml/kg;辛伐他汀组给予辛伐他汀20 mg·kg-1·d-1,均灌胃8周,每日1次。比较各组大鼠AI、胸腺指数、膝关节伸直方向活动度和血清白介素-1β(IL-1β)、基质金属蛋白酶(MMP)-3、MMP-9水平的变化。结果其他三组AI和血清IL-1β、MMP-3、MMP-9水平高于正常对照组(P均<0.05),胸腺指数低于正常对照组(P均<0.05);模型组和阳性对照组膝关节伸直方向活动度低于正常对照组(P均<0.05)。除正常对照组外的三组中,按模型组、阳性对照组、辛伐他汀组顺序,AI及血清IL-1β、MMP-3、MMP-9水平依次降低,胸腺指数依次增高,膝关节伸直方向活动度依次增大,各指标三组间两两比较差异均有统计学意义(P均<0.05),即辛伐他汀组AI和血清IL-1β、MMP-3、MMP-9水平最低,胸腺指数最高,膝关节伸直方向活动度最大。结论辛伐他汀能改善膝骨性关节炎大鼠的症状,可能通过降低大鼠血清IL-1β、MMP-3、MMP-9水平,调节机体细胞因子和保护软骨基质而发挥作用。Objective To investigate the effects of simvastatin on arthritis index( AI),thymus index( TI),knee joint range of motion( ROM) in extension direction and serum cytokines levels in rats with knee osteoarthritis. Method Sixty rats were randomly divided into normal control group,model group,positive control group and simvastatin group( n = 15 each). In addition to normal control group administered by gavage with same volume normal saline,the osteoarthropathy model was established in rest 3 groups by injecting papain into the knee joint cavity of rats. After successful modeling,voltaren suspension( 10 ml / kg) was given in positive control group; simvastatin( 20 mg·kg^-1·d^-1) was given in simvastatin group;same volume normal saline was given in model group. All rats were administered by gavage medication once a day for eight weeks. The AI,TI,knee joint ROM in extension direction and serum interleukin-1β( IL-1β),matrix metalloproteinase( MMP)-3,MMP-9 levels were compared among all groups. Results Compared with normal control group,AI and serum IL-1β,MMP-3,MMP-9 levels significantly increased( All P〈0. 05); TI significantly decreased in other groups( All P〈0. 05); knee joint ROM in extension direction significantly decreased in model group and positive control group( All P〈0. 05). In all groups except normal control group,AI and serum IL-1β,MMP-3,MMP-9 levels decreased in turn; TI increased in turn; knee joint ROM in extension direction increased in turn all in the order of model group,positive control group,simvastatin group; namely,AI and serum IL-1β,MMP-3,MMP-9 levels were lowest; TI was the highest; knee joint ROM in extension direction was the largest in simvastatin group. There were significant differences in aforementioned inde-xes for pairwise comparison among model,positive control and simvastatin groups( All P〈0. 05). Conclusions Simvastatin can improve the symptoms of knee osteoarthritis rats and it maybe through decreasing serum IL-1β,MMP-3,MM
