膝骨关节炎关节镜清理术并发症病因分析及对策

摘　　要：目的探讨膝骨关节炎行关节镜清理术并发症的发生原因、转归及对策。方法自2008-02-2013-08对698例膝骨关节炎行关节镜清理术，分析其并发症发生的原因。结果所有患者均获得随访8-67个月，平均51．2个月。共42例发生并发症，发生率6．01％。其中关节积液肿胀9例，关节血肿5例，关节软骨损伤3例，器械断裂1例，隐神经髌下支损伤2例，滑膜疝2例，关节感染2例，术后疼痛加重行人工全膝关节置换6例，关节周围肌腱慢性炎症7例，关节活动受限3例，切口不愈合2例。42例均给予积极相应治疗，40例疗效满意。结论严格把握膝骨关节炎手术适应证，术前术中的正确诊断、熟练的手术操作和对并发症的及时正确处理是减少膝关节镜清理术并发症发生以及使其转归良好的关键。Objective To investigate the etiology, results and treatment of the complications of the knee arthroscopic debridement operation in osteoarthritis. Methods A total of 698 patients with knee osteoarthritis underwent knee arthroscopic dcbridement operation in our department from Feb. 2008 to Aug. 2013. Results All patients were followed up for 8 to 67 months, with an average of 51.2 months. Forty two patients had complications. The incidence of complications was 6.01%. Nine cases had severe hydrarthrosis, 5 cases haemarthrosis, 3 cases articular cartilage injury, 1 case instrument broken, 2 cases the infrapatellar branch of saphenous nerve injury, 2 cases synovial hernia, 2 cases joint infection, 6 cases received total knee arthroplasty for severe pain, 7 cases had chronic inflammation of the periarticular tendon, 3 cases joint activity limitation, 2 cases poor healing of incision. Forty of the 42 patients with complication got satisfied results after corresponding treatment. Conclusion It is important to decrease complications and acquire good outcomes of arthroscopic operation to strictly observe the operation indication, correctly diagnose, skillfully perform operation and treat complications in time.
