多钢板联合MIPPO技术治疗“四柱损伤”Pilon骨折的疗效

摘　　要：目的探讨多钢板联合微创经皮钢板内固定术（MIPPO）治疗“四柱损伤”Pilon骨折的疗效。方法自2013-01-2015—05运用多钢板联合MIPPO技术治疗28例“四柱损伤”Pilon骨折，骨折按Rtiedi—Allgower分型均为Ⅲ型。结果术后26均获平均18．4（4—28）个月随访，失访2例。术后平均2．3（1．5～4）个月可见到明显的骨痂生长，部分负重，骨折愈合时间平均4．2（3—8）个月，内固定物无松动及断裂，骨折未见移位，可以弃拐行走。疗效按Mazur评分标准，优14例，良8例，可3例，差1例，优良率84．6％。结论采用多钢板联合MIPPO治疗“四柱损伤”Pilon骨折具有固定牢靠，可以早期进行功能锻炼，创伤小，愈合率高，软组织并发症少等优点。Objective To evaluate the efficacy of multi plates combined with MIPPO technique for the treatment of ＂four- column injury＂ Pilon fractures. Methods From January 2013 to May 2015, 28 patients with ＂four-column injury＂ Pilon fractures were treated with multi plates combined with MIPPO technique. Based on the Rtiedi-Allgower classification system, all of them were divided into type Ⅲ. Results Twenty six patients were followed up for d to 28 months（average, 18.4 months） after surgery, and 2 patients were lost. All of patients obtained obvious callus growth after 1.5 to 4 months （average, 2.3 months） and walked with partial weight bearing. All patients began to walk with full weight-bearing after the fractures healed in 3 to 8 months （average~ 4.2 months） without loosening and breaking of the internal fixation and displacement of fractures. According to the Mazur＇s score classification, the excellent result was achieved in 14 patients, good in 8, acceptable in 3 and failure in 1, the excellent and good rate was 84.6%. Conclusion The treatment of ＂four-column injury＂ Pilon fractures by multi plates combined with MIPPO technique has the advantages such as more stable fixation, earlier functional exercise, smaller incision, higher healing rate and less complications of soft tissue.
