摘 要:目的通过对急性心肌梗死(AMI)患者血清低氧诱导因子-1α(HIF-1α)的检测与分析,评价其在心肌梗死早期的诊断价值及临床应用的可能性。方法选取胸痛发作6 h内急诊入院患者,经冠状动脉造影及心电图检查结果分为3组:S-T段抬高型心肌梗死患者入AMI组(30例);心绞痛患者入AP组(32例);冠状动脉造影及心电图显示正常的人群为N组(25例);采集3组血清,分别采用生化分析仪、化学发光法及酶联免疫试剂盒测定肌酸激酶同工酶(CKMB)、肌钙蛋白I(CTn I)、N末端脑钠肽前体(NT-pro BNP)及HIF-1α的血清含量。结果 AMI组的HIF-1α、CKMB、CTn I及NT-pro BNP含量均明显高于N组与AP组(P<0.01);AMI组的HIF-1α含量与CKMB,CTn I及NT-pro BNP均呈正相关;受试者工作特征(ROC)曲线分析显示HIF-1α的曲线下面积(AUC)为0.884,以100.5 pg/ml为HIF-1α的诊断临界点时,对AMI诊断的灵敏度为83.33%,特异度为88.89%。结论血清HIF-1α可能是一种新的可用于AMI早期诊断的生物标志物。Objective To determine and analyze serum hypoxia-inducible factor-1α (HIF-1α ) in patients with acute myocardial infarction (AMI), and to evaluate its potential diagnostic value and clinical use in early AMI. Methods Patients with chest pain and hospitalized emergently within 6 h were included in the study. All patients were divided into 3 groups according to the findings of coronary angiography and electrocardiogram (ECG): patients with acute S-T segment elevation myocardial infarction were included in the AMI group (n=30); patients with angina pectoris (AP) were included in the AP group (n=32); controls with normal coronary angiography and ECG were included in the N group (n=25). The serum in the three groups was collected. The serum levels of CKMB, CTnI, NT-proBNP and HIF-1α were determined by biochemical analyzer, chemiluminescence and enzyme linked immunosorbent assay (ELISA) kit, respectively. Results The levels of HIF-1α , CKMB, CTnI and NT-proBNP in the AMI group were significantly higher than those in the N group and AP group (P〈0.01). The HIF-1α levelwas positively correlated with CKMB, CTnI and NT-proBNP in the AMI group. The receiver operating characteristic (ROC) curve showed that the area under the curve (AUC) of HIF-let was 0.884. When the cutoff point of HIF-let was 100.5 pg/ml, the sensitivity was 83.33% and the specificity was 88.89% in the diagnosis of AMI. Conclusion Serum HIF-1α may be a novel biomarker for early diagnosis of AMI.
