三阴交穴-原发性痛经的即刻镇痛效应特点

摘　　要：国家重点基础研究发展计划(973计划)课题"调控胞宫的经穴效应特异性"对三阴交穴治疗原发性痛经的即刻镇痛效应及其机制、相关特点分别进行了探索.结果发现,分别针刺三阴交穴和相同脊髓节段支配的血海穴、悬钟穴、非经非穴,均有即刻镇痛效应.三阴交穴的效应优于血海穴(相同经脉的非特定穴)、悬钟穴(与病位非相关经脉经穴)、非经非穴.针刺三阴交穴治疗原发性痛经的机制是通过激活外周与中枢的相关镇痛通路而实现的.在外周通过调节靶器官痛相关物质的代谢,进而抑制子宫过度收缩、改善子宫微循环状态、增强靶器官抗氧化应激的能力;在中枢通过激活各级水平的内源性痛调制系统,促进了相关镇痛物质的合成与释放,最终调节靶器官的失衡状态,缓解疼痛.三阴交穴的即刻镇痛效应会受到患者证型和针刺介入时机的影响.针刺三阴交穴不仅对原发性痛经具有治疗效应,其穴区的电阻失衡还具有反应该疾病变化的特点.研究结论有助于较为全面地揭示单穴的经穴效应特点,进而为经穴效应特异性的理论内涵提供依据.The program ＂Research on Acupoint Specificity in Regulating Uterus＂ supported by the National Basic Research Program of China（973 Program）（No. 2006CB504503） studied the immediate analgesia effects of Sanyinjiao（SP6） acupuncture in primary dysmenorrhea（PD）, its mechanism and interrelated characteristics. In this research, we observed that inserting in the same nerve segment acupoints [Sanyinjiao（SP6）, Xuehai（SP10）, Xuanzhong（GB39）, non-acupuncture point（NAP）] separately had immediate analgesia effects. Acupuncture at SP6 can immediately relieve menstrual pain compared with SP10（non-special acupoint at the same meridian with SP6）, GB39（non-related meridian acupoint） and NAP. The mechanism of treating PD by acupuncture at SP6 is to activate analgesia passages in peripheral target organ and central nervous system： in peripheral target organ, pain-related substance metabolism is regulated to suppress uterine contraction, improve the microcirculation of the uterus, and enhance the ability of anti-oxidative stress; in the central nervous system, endogenous pain modulation system is activated to promote the synthesis and release of analgesia substances, modulate the imbalance of target organs, and relieve pain finally. The immediate analgesia effects of SP6 are affected by traditional Chinese medicine syndrome type and acupuncture intervention time. In addition to the treatment effect in PD, SP6 is also a reaction point of PD. The results of this study are useful to reveal single point effect and its characteristics, thus providing the basis for the theoretical connotation of the specificity of acupoints.
