摘  要:背景:生物材料的发展为心肌梗死的治疗提供了新的思路,目前已有纤维蛋白胶、基质胶、细胞外基质乳液、藻酸盐等多种生物材料用于心肌再生的治疗。目前有关血小板纤维蛋白支架治疗心肌梗死的研究不多。目的:探讨血小板纤维蛋白支架对大鼠心肌梗死的治疗作用。方法:采用血小板纤维蛋白胶制备血小板纤维蛋白支架。将40只心肌梗死模型大鼠随机分为2组,血小板纤维蛋白支架组在大鼠的梗死心肌四周注射血小板纤维蛋白胶200μL,对照组在心肌梗死四周相同的位置注射等量的DMEM。检测血小板纤维蛋白支架释放血管内皮生长因子、肝细胞生长因子、胰岛素样生长因子1等细胞因子的表达情况,观察血小板纤维蛋白支架植入后心肌梗死大鼠内皮细胞和心肌细胞的生成、心肌细胞凋亡、血管密度及其心脏功能。结果与结论:(1)血小板纤维蛋白支架被成功制备,在纤维蛋白支架培养的第14天时血小板纤维蛋白支架释放的血管内皮生长因子为(115.7±19.45)ng/L,肝细胞生长因子为(23.98±12.32)ng/L,胰岛素样生长因子1为(288.4±16.74)ng/L;(2)血小板纤维蛋白支架组的v WF阳性细胞数及a-SA阳性细胞数均明显高于对照组(P<0.05),血小板纤维蛋白支架组的TUNEL阳性细胞数低于对照组(P<0.05);(3)血小板纤维蛋白支架组的毛细血管密度及有血流的毛细血管均明显高于对照组(P<0.05);(4)血小板纤维蛋白支架组的左心室舒张末期容积和左心室收缩末期容积均明显低于对照组(P<0.05);(5)结果提示,血小板纤维蛋白支架能够促进内皮细胞、心肌细胞以及毛细血管的生成,改善心脏功能,血小板纤维蛋白支架对心脏的保护作用可能与其分泌的细胞因子有关。BACKGROUND:The development of biomaterials provides a new idea for myocardial infarction treatment, and so farthere are numerous biomaterials, such asfibrin glue, matrigel, emulsion of extracelular matrix and alginate,for myocardial infarction. But there is a lack of studiesabout the platelet-fibrin scaffold for myocardial infarction. OBJECTIVE:To explore the effect oftheplatelet-fibrin scaffold on rat myocardial infarction. METHODS:Platelet-fibrin was usedto prepare aplatelet-fibrin scaffold, and 40 rat models of myocardial infarction were randomly divided into two groups:200 μL platelet fibrins were injected into the tissues around the rat infarcted myocardium as platelet-fibrin scaffold group, and the same amount of DMEM was injected into the tissues around the rat infarcted myocardium as control group. Subsequently, expressions of vascular endothelial growth factor, hepatocyte growth factor and insulin-like growth factor 1released fromthe scaffold were detected.Production ofendothelial cels and cardiomyocytes, cardiomyocyte apoptosis,capilarydensity and cardiac functionin ratsafter implantationwere observed. RESULTS AND CONCLUSION:The platelet-fibrin scaffold was successfuly prepared. The expression of vascular endothelial growth factor, hepatocyte growth factor and insulin-like growth factor 1 in the scaffold groupwas (115.7±19.45), (23.98±12.32) and (288.4±16.74) ng/L, respectively. The number of von Wilebrand factor-positive cels and a-SA-positive cels in the platelet-fibrin scaffold groupwassignificantly higher thanthat inthe control group (P〈 0.05). The number of TUNEL-positive cels in the platelet-fibrin scaffold group wassignificantly lower than thatinthe control group (P〈 0.05). The capilary density and capilaries with blood flow in the platelet-fibrin scaffold group were significantly higher than thoseinthe control group (P〈 0.05).
